Home Carers - Home Care Team and Dame Alice Court
Salary: £17,007 - £17,391 pro rata per annum
Home Care Team:
To provide support, encouragement and physical assistance (when required) to
adults within their own home to increase their confidence and independence.
To assist with the facilitation of discharges from hospital and respite placements
to ensure a timely and safe transfer to the community.
To assist with the prevention of admission to hospital by providing skilled level of
support, under guidance of Coordinator and/or Locality Manager.
To ensure that all clients have an outcome focused support plan, which is
updated as their independence increases.
Dame Alice Court:
To have responsibility for offering care in accordance with the practices and procedures
of the authority to customers whose needs have been assessed under care
management. Duties will include physical care and domestic duties and, where
applicable, social duties, aimed at creating a supportive homely atmosphere where
customers can achieve maximum independence.
