DWC Finalizes New Subsequent Injury Fund Forms and Offers Electronic Submission Option

The Division of Workers’ Compensation (DWC) has finalized forms to help insurance carriers submit requests for reimbursement from the Subsequent Injury Fund (SIF). The forms will help expedite an insurance carrier’s request for reimbursement by reducing requests from DWC for additional information.

DWC encourages SIF requests be submitted electronically, by fax at 512-804-4759 or secure electronic file transfer.
