Copies of Designated Doctor Request Now Provided With Division Order

On September 1, 2016, the Division of Workers’ Compensation (DWC) will begin sending all parties a copy of the Request For Designated Doctor (DWC Form-032) along with the Commissioner’s order for a designated doctor exam. If the ordered examination is a result of a Presiding Officer’s Directive to Order Designated Doctor Exam (“POD”), a copy of that request will be attached.

For designated doctors and their administrative services companies, the exam order and the copy of the DWC Form-032 or POD will be faxed together to the number on the designated doctor’s TXCOMP Provider profile.

These changes should streamline the process and provide a copy of the forms that DWC used to schedule the exam.

Parties must continue to provide a copy of the request to each party who did not request the designated doctor exam when submitting the request to DWC, as required by Division rule.
