Dear Neighbors and Friends,

I hope that you and your loved ones are doing well, staying healthy, and looking out for your neighbors and friends during this past week.

As you’ll see throughout this newsletter in the reports, links, and graphs, we are clearly seeing big drops in COVID case numbers as the forecasts predicted.  This has led the Governor to announce that our indoor masking requirements are on track to be rescinded at the end of March.  According to recent forecasts from OHSU, that’s when our hospitalization rate should be back down to where it was prior to the Omicron surge.  The Governor’s medical advisors believe that this is the key metric to be used to signal our entry into a safer period, at least for those who are fully vaccinated.

For now, the CDC remains more cautious.  They and others are concerned that announcements such as this—promising a change in requirements in six weeks--will in fact exacerbate a sense of impatience and induce many people to begin to change their behavior immediately.  This is a legitimate concern, as we can see in this week’s OHSU forecast: if people really do hold off on changing their behavior till the end of March, it will have negligible impact on the downward course of the disease; however, if they start to change their actions immediately, we’ll see an immediate slowdown in the downward trajectory.  Let’s hope people can remain patient a little longer.

The fact is that even with the big declines that we're seeing in new cases, the rates are still extremely high.  Omicron remains incredibly infectious, including for people who are fully vaccinated (the share of new reported "breakthrough" cases is actually increasing, in part because of improved reporting methods by OHA. However, fortunately, most of these breakthrough cases remain relatively mild.  For those who are unvaccinated, however, COVID has become increasingly deadly.  

Shifting gears to legislative news--as you can imagine, it has been an incredibly packed week in the Legislature, with lots of action in committee and on the floor (including a senator evicted for refusing to wear a mask and the House looking at the return of bills needing to be read aloud).  Today marked the deadline for more than half the bills to be voted out of committee, with the remainder facing a deadline on Monday. As always at this point in the session, most of the introduced bills have already reached the end of the road for this session.

I have lots to report, but not enough time or energy this evening.  So plan on receiving an additional newsletter this weekend with updates, descriptions, and predictions. By the way, we’ll be looking at four floor sessions next week—Monday through Thursday.

We also learned yesterday that Oregon’s economy, business income, and wages continue to surge at record levels, despite the pandemic. This has led our state economists to revise their projections for Oregon’s tax receipts upwards by close to a billion dollars.  I’ll go into the details and the implications of this revision in this weekend's newsletter.

OREGON CORONAVIRUS AND CORONAVIRUS RESPONSE UPDATE

  • New COVID Cases: OHA reports 3,176 new COVID cases today, vs. 5,417 last Thursday. That’s an average of 3,219 per day for the last week (vs. 5,181 per day the previous week). The cumulative number of cases in Oregon since the beginning of the pandemic is 672,923.
  • Variant COVID Cases:
    • OHA is now showing us not only this week’s variant cases but the growth of the different variants in Oregon over time. You can see in the graph above that Omicron remains responsible for nearly all the cases in this state This graph is updated every Wednesday.
    • OHA’s Variant Dashboard provides current and new variant case numbers for the state as a whole but also for different parts of the state. It too is updated each Wednesday.
  • Positive Test Results: OHA reported 3,619 positive test results today (vs. 5,998 last Thursday). That’s an average of 3,920 per day for the 7 days since the last newsletter (vs. 6,430 per day the previous week). The cumulative total of positive test results since the beginning of the pandemic is now 858,994. 886.436
  • Total Tests: OHA reported an additional 34,473 tests today (vs. 45,473 last Thursday). That’s an average of 24,872 per day for the last week (vs. 31,932 for the previous week). Our cumulative total of reported tests is 10,130,262.
  • Positivity Rate: The test positivity ratio for Oregon today was 12.9% (vs. last week’s 17.4%). That’s an average of 15.8% per day for the last week (vs. 20.2% last week).
  • Hospitalization Information:
    • Patients Currently with Confirmed COVID-19: 1,007 (80 fewer than last newsletter)
    • ICU Patients Confirmed w COVID-19: 172 (11 fewer than last newsletter.)
    • Confirmed COVID-19 Patients Currently on Ventilators: 76 (13 fewer than last newsletter)
    • Available Ventilators: 842 (18 fewer than last newsletter).
  • Hospital Capacity:
    • Available Adult ICU Beds: 54 (8% availability) (3 fewer than last newsletter).

  • Deaths: I’m sorry to report 22 additional COVID deaths today (vs. 18 last Thursday).  That’s an average of nearly 23 per day for the week since the last newsletter (vs. 16 the previous week). The total number of COVID deaths in Oregon is 6,344.
  • Vaccinations:
    • Today, OHA reported that 8,683 new doses of COVID-19 vaccinations were added to the state immunization registry Feb. 9. Of that total, 740 were initial doses, 978 were second doses and 2,310 were third doses and booster doses. The remaining 4,509 were administered on previous days but were entered into the vaccine registry Feb. 9.
    • The seven-day running average is now 7,159 doses per day.

  • California will lift its indoor mask mandates . starting next week. However, their governor’s plan limits the change to those who are fully vaccinated.

  • Here’s the latest Update from Dr. Wen. It focuses on the decision by several Democratic governors (including our own) to begin relaxing COVID requirements now or in the near future. She largely agrees with these decisions.

Weekly County Report: Big Reductions Around the State

The latest OHA county report, showing increases/decreases in COVID spread for each county and for the state as a whole, was released on Monday. It shows case counts and rates for the last four weeks, including the week that ended Sunday, February 6.

The Omicron transmission numbers reported this week continue to be extremely high, but they are going down for the state as a whole, and in every county.

The statewide infection rate has gone from to 1,049.5 per 100K in the last report to 689.4 in this one; the test positivity rate has also gone down, from 22.6% last week to 18.2% this week.

As you’ll see in the report, the rates remain extremely high everywhere, but they are definitely coming down.

 

OHA Weekly Report: Big Drop in New Oregon Cases, Big Increase in Hospitalizations

OHA’s COVID-19 Weekly Report released yesterday, showed a continued steep decline in weekly cases, but sharply higher hospitalizations.

  • OHA reported 28,378 new cases of COVID-19 during the week of Monday, Jan. 31 through Sunday, Feb. 6 – a 35% decline from last week’s 43,606.
  • There were 964 new COVID-19-related hospitalizations, a 30% increase over the previous week’s 739.
  • COVID-19-related deaths declined slightly from 147 to 144.
  • Reported COVID-19 test results dropped by 22%. There were 194,270 tests administered, vs. last week’s 249,556.
  • Test positivity declined from 22.6% to 18.2%.

The COVID-19 Weekly Outbreak Report shows 341 total active outbreaks in care facilities, senior living communities and congregate living settings.

The Latest Breakthrough Report: Share of Breakthrough Cases Continues to Rise, While Severe Cases Continue to Be Among the Unvaccinated

OHA’s most recent update on COVID-19 breakthrough cases, released today, reported 29,411 cases of COVID-19 during the week of Jan. 30 to Feb. 5.

Of those cases, 15,020, or 51.1% (vs. last week’s 71.2%), were unvaccinated people and 12,778, or 43.41%, were vaccine breakthrough cases. Among the vaccine breakthrough cases, 4,637, or 36.3%, were fully vaccinated and boosted (vs. last week’s 31.4%).

79 breakthrough cases were residents of care facilities, senior living communities or other congregate care settings (down from 128 last week).

To date, there have been 145,313 COVID-19 vaccine breakthrough cases in Oregon, out of a total of 672,923.

Although the majority of COVID cases continue to involve unvaccinated people, the difference between vaccinated and unvaccinated continues to narrow.  The report shows that the rate of COVID-19 in unvaccinated people is almost double that of fully vaccinated people (down from three times last week), and 3.4 times higher than in fully vaccinated and boosted people (down from 6.5 times).

Where the vaccines continue to really make a difference is in preventing serious COVID cases.  Here, the gap between vaccinated and unvaccinated individual is increasing, not narrowing.  Nearly all breakthrough cases show no symptoms, or symptoms that are relatively mild. The percentage of breakthrough hospitalizations and deaths continues to go down.  To date, only 2.6% of the small percentage of vaccine breakthrough cases have been hospitalized (down from 2.7%), and just 0.6% have died (same as last week). Nearly all the COVID patients in ICUs and on ventilators have been unvaccinated.

To date, nearly 3 million Oregonians are fully vaccinated.

Note from OHA:

This week and going forward, the weekly COVID-19 Breakthrough Report includes several changes.

In order to identify breakthrough cases, all cases reported to public health are automatically matched with Oregon’s statewide immunization database to verify COVID-19 vaccination status. As case counts have increased dramatically during the Omicron surge, the database has been unable to match the entire volume of reported cases. This has created a portion of recent COVID-19 cases with unknown COVID-19 vaccination status.

The following changes to the weekly breakthrough report were made to better understand the spread of COVID-19 among those who are fully vaccinated:

  • Categorizing cases not yet matched with the statewide immunization database as those with “unknown vaccination status.”
  • Adding additional columns in Table 1 with the number and percentage of cases with known vaccination status by week.
  • Updating Figure 1 so the “unvaccinated cases” category excludes cases with unknown vaccination status. This change provides more accurate information on COVID-19 cases rates over time among those who are not fully vaccinated.
  • Updating Table 3 so the “unvaccinated cases” category excludes cases with unknown vaccination status.

OHA is working to identify vaccination status for all reported cases in a timely manner.

 

New OHSU Forecast Confirms Oregon’s Omicron Peak Has Been Reached

The current OHSU forecast report, published today, uses data provided by OHA and others that project how fast the virus may spread in the population and provides projections on possible outcomes, including infection rates and impacts on hospital capacity.  The lead author is Dr. Peter Graven, Director of OHSU’s Office of Advanced Analytics

This forecast is similar to the one from last week, which led to the Governor’s decision to lift mask mandates at the end of March.   It again shows that we have passed the peak and are starting to see numbers go down steadily for new COVID cases and more slowly for hospitalizations.  COVID cases will likely continue to go down more quickly than hospitalizations.  Hospitalizations should return to their pre-surge level (400 statewide) by the end of March.

Here are key observations in this week’s report:

  • As of Feb. 9, 1,042 people were in Oregon hospitals. The number has been relatively flat over the last three weeks.
  • The number of people in hospitals is flat or falling in most regions of the state, but it's still rising in Region 3 (Coos, Curry, Douglas and Lane counties).
  • Case counts and positive tests are both decreasing.
  • Oregonians continue to wear masks, but other behaviors are moving toward pre-omicron levels.
  • An estimated 1 million Oregonians remain susceptible to infection because they are not vaccinated or have not been infected before.
  • As of Feb. 9, 27% of occupied ICU beds had COVID patients in them statewide, a decrease from 32% the week before.
  • There were 27 children in Oregon hospitals. The level can shift day to day, but a sustained increase is not expected.
  • Flu cases remain low; 35 were reported, compared with 1,008 in a similar week two years ago. 
  • The forecast for COVID-19 deaths has been revised from last week because of a mistake in an equation. The new forecast shows a higher number of deaths expected during the omicron surge.
  • The forecast shows that Oregon has most likely seen its peak, and should begin a decline in the next week.
  • The model shows that lifting restrictions today could result in a jump in patients needing hospital care. It predicts a relatively small impact if restrictions remain in place until the end of March.

 

And the Deaths:

Here is information about the 163 additional deaths that have been reported since last Thursday’s newsletter, vs. 114 the week before. 

Of those 163, only 118 actually occurred in January or February, vs. 74 last week. The daily reported deaths often include reports coming in late, and in some cases very late. So, these daily reports are always subject to revision.

OHA does have a dashboard that shows week-over-week numbers of deaths by actual date of death, not when the deaths were reported (i.e., they go back and plug those late-reports into the weeks they actually occurred.  You can find that dashboard here. It’s updated once a week.
