January 27th COVID-19 Update

Dear Neighbors and Friends,

I hope that you and your loved ones are doing well, staying healthy, and looking out for your neighbors and friends during this past week.

As you’ll see in tonight’s newsletter, here in Oregon we’re at or approaching our Omicron peak.  This is pretty much in line with what the OHSU forecasters had predicted.  As we’re seeing elsewhere around the country, the peak is occurring sooner in the more populous Portland-area counties, where Omicron hit first, but we should start seeing rates come down in the rural areas as well. 

Unfortunately, with their lower vaccination rates, the rural counties are going to see much higher hospitalization rates in the coming weeks—we’re now learning just how dramatically higher the hospitalization rate is for those not fully vaccinated and boosted: nearly 50 times higher for seniors.  Things are going to remain extremely challenging for the next month or so.

And of course hitting the peak doesn’t mean the end of new cases—there are potentially as many new cases to come on the way down from the peak as there were on the way up.

While our overall death rates from COVID are among the lowest of any state, Oregon did hit another grim marker this week: more than 6,000 deaths from this pandemic so far.  Every one a loss, someone gone before their time. My thoughts are with them and their families.

It’s late Thursday evening, and there’s still no new OHSU forecast posted on their website.  Sorry about that.  I’ll update you next week.

This is of course an extremely busy time for legislators getting ready to start the 2022 session. We’re all working on our personal priority bills, refining, scheduling, and building support for them. (Here’s a just-released press release on one of mine: SB 1567, which calls for immediate action to address the certain dangers posed by the fuel tanks on the banks of the Willamette, the “Critical Energy Infrastructure Hub.” You’ll be hearing lots more about it and other priorities in the coming weeks.)

By the time you get next week’s newsletter, the Legislature will be finishing up the first week of its five-week session.  The 2022 session convenes on Tuesday morning, when all bills will be “first-read” and referred to committees.  If you go to the Oregon Legislative Information System (olis@oregonlegislature.gov), you can find all the bills and all the committees.  Starting Tuesday you can watch floor sessions and hearings, sign up to be notified of committee agendas and action on individual bills.  You can sign up to speak to committees in public hearings and submit written testimony on any bill that’s being heard.  You can also find a wealth of “how-to’s” on the main legislative website—oregonlegislature.gov.  I encourage you to check it out.

You can of course visit the Capitol in person now, as long as you’re wearing a mask.  As I’ve mentioned, you’ll need to be prepared to go through the new Xray and metal detectors, and most legislators won’t be allowing you up into their offices for now.  All of our committees will again be meeting virtually via Microsoft Teams, though we will continue to meet periodically in person for votes on the floor.  Next week we expect to have floor sessions on Tuesday and Thursday.  Then the following week it will likely be every day.

If we do see the Omicron wave subside dramatically early in February, we may be able to have more in-person contact, but for now we’re taking it one week at a time.  We're obviously hoping to keep things as safe as possible.  Hopefully, all legislators will be on the same page with that, but of course with the politicization of pandemic response, that may be too much to expect.  I’ll give you a report next week.

OREGON CORONAVIRUS AND CORONAVIRUS RESPONSE UPDATE

  • New COVID Cases: OHA reports 7,871 new COVID cases today, vs. 10,034 last Thursday. That’s an average of 7,594 per day for the last week (vs. 7,890 per day the previous week). The cumulative number of cases in Oregon since the beginning of the pandemic is 613,121.
  • Variant COVID Cases:
    • OHA is now showing us not only this week’s variant cases but the growth of the different variants in Oregon over time. You can see in the graph above that Omicron is now responsible for nearly all the cases in this state This graph is updated every Wednesday.
    • OHA’s Variant Dashboard provides current and new variant case numbers for the state as a whole but also for different parts of the state. It too is updated each Wednesday.
  • Positive Test Results: OHA reported 9,730 positive test results today (vs. 11,663 last Thursday). That’s an average of 9,422 per day for the 7 days since the last newsletter (vs. 10,125 per day the previous week). The cumulative total of positive test results since the beginning of the pandemic is now 813,983.
  • Total Tests: OHA reported an additional 41,227 tests today (vs. 43,195 last Thursday). That’s an average of 39,185 per day for the last week (vs. 43,327 for the previous week). Our cumulative total of reported tests is 9,732,637.
  • Positivity Rate: The test positivity ratio for Oregon today was 23.6% (extremely high, but down from last week’s incredible 27.0%). That’s an average of 24.0% per day for the last week (vs. 23.4% last week).
  • Hospitalization Information:
    • Patients Currently with Confirmed COVID-19: 1,130 (149 more than last newsletter)
    • ICU Patients Confirmed w COVID-19: 169 (27 more than last newsletter.)
    • Confirmed COVID-19 Patients Currently on Ventilators: 79 (12 more than last newsletter)
    • Available Ventilators: 852 (3 fewer than last newsletter).
  • Hospital Capacity:
    • Available Adult ICU Beds: 59 (9% availability) (14 more than last newsletter).
    • Other Available Adult Beds: 266 (6% availability) (15 more than last newsletter).
    • Available Pediatric ICU Beds: 6 (15% availability) (same as last Thursday).

  • Deaths: I’m sorry to report 19 additional COVID deaths today (vs. 8 last Thursday).  That’s an average of nearly 7 per day for the week since the last newsletter (vs. 20 the previous week, an inflated number due to late reporting of deaths that occurred a while ago). The total number of COVID deaths in Oregon is 5,916.6,067.
  • Vaccinations:
    • Today, OHA reported that 13,523 new doses of COVID-19 vaccinations were added to the state immunization registry Jan. 26. Of that total, 1,200 were initial doses, 1,029 were second doses and 4,924 were third doses and booster doses. The remaining 6,235 were administered on previous days but were entered into the vaccine registry Jan. 26.
    • The seven-day running average is now 10,859 doses per day (down from 14,865 a week ago).
    • Total doses administered: 7,154,999
      • 4,026,046 Pfizer/Comimaty vaccines
      • 106,062 Pfizer Pediatric vaccines
      • 2,645,448 Moderna vaccines
      • 264,277 Johnson & Johnson vaccines
    • Progress towards Governor’s goal of 2 million boosters by the end of January (announced Dec. 17): 62,801
      • When the challenge began, 949,749 people had received a booster dose.
      • Since then, 478,497 Oregonians have received a booster (up by 62,801 from last week).
      • As of today, Oregon needs 521,503 people to get a booster to reach the goal.
    • Total Oregonians vaccinated so far: 3,120,082 (up by 16,392 from last week)
      • 2,820,422 now fully vaccinated with two doses (up by 11,249 from the previous week)
    • So far, 75.9% of all Oregonians have received at least one dose

  • Though COVID numbers seem to have peaked in the Portland metro area, Multnomah County public health officials believe that by the end of February, half of all County residents will have been infected. That’s just incredible.
  • We’re fortunately seeing a steep reduction in COVID hospitalizations in the Northeast. The West continues to see big increases.  Here's more.
  • California as a whole appears to have reached its Omicron peak.
  • Oregon is seeing increasing numbers of pediatric COVID hospitalizations. It’s the highest so far for children in the pandemic.

  • The New York Times’ David Leonhardt recently wrote a piece called "Two COVID Americas," about a recent poll that shows points of convergence and points of divergence regarding attitudes about COVID, vaccines, and appropriate responses.  Leonhardt points out the irony that in these days of Omicron those who are vaccinated and boosted are overly cautious about getting out and about, given the extremely low odds they face of serious harm from the disease, while those who are unvaccinated are dangerously careless, given the very high risk of serious disease and even death that they face.  Here's the transcript of a recent interview he did on the subject with The Daily’s Michael Barbaro.  Thought-provoking.
  • In fact, recent studies show that unvaccinated seniors are  49 times more likely to be hospitalized from COVID than those who’ve been vaccinated and boosted. And here's more on the recent studies.

  • Here’s the latest Checkup with Dr. Wen. It focuses on recent research showing that COVID vaccines do NOT negatively affect fertility.
  • Conspiracy theorists who are banned from using social media are finding different ways to get their messages out.

Rental Assistance Back Up and Running

You may remember that the Legislature allocated extra money back in December to protect more people affected by COVID from being evicted from their rentals.  Even though more than $240 million had been sent to landlords to cover back payments, federal support has run out and needed to be augmented by money from the state.

Weekly County Report: Starting to Level Off

The latest OHA county report, showing weekly increases/decreases in COVID spread for each county and for the state as a whole, was released on Monday. It shows case counts and rates for the last four weeks, including the week that ended Sunday, January 23.

The Omicron transmission numbers reported this week continue to be extremely high.  However, they are only slightly higher than last week’s.

The state as a whole remains at EXTREME HIGH RISK of infection. The infection rate has gone from 1303.1 per 100K last week to 1,335.8 1,063.4 per 100K this week to; the test positivity rate, however, has continued to rise substantially, from 22.0% last week to 24.5 this week.

As we’ve seen around the country, the Omicron peaks hit the populous areas first, and so did the declines.  The Portland-area counties are driving the statewide reduction rates, as they are seeing their infection rates starting to decline (though still extremely high).  The rural counties are still seeing increases overall, though the rates of increase are slowing down. 

 

Oregon Cases Decline, OHA Weekly Report Shows

OHA’s COVID-19 Weekly Report released yesterday showed a decline in weekly cases, but hospitalizations are still rising sharply, and test positivity rates remain extremely high.

  • OHA reported 47,361 new cases of COVID-19 during the week of Monday, Jan 17, through Sunday, Jan. 23 – a 9.5% decline from last week’s pandemic high.
  • There were 638 new COVID-19-related hospitalizations, a 45% increase over the previous week’s 445, and the highest since the Delta surge peaked at 1,036 hospitalizations. (However, in line with what we’ve been seeing with Omicron, new ICU admissions increased at a much lower rate.)
  • COVID-19-related deaths fell to 59, from 83 the previous week.
  • Reported COVID-19 test results dropped 9.9% to 288,232 for the week of Jan.16 to Jan.22, down from last week’s 320,210.
  • Test positivity rose from 22.5% to 25%.

Today’s COVID-19 Weekly Outbreak Report shows 273 active COVID-19 outbreaks in senior living communities and congregate living settings, another increase from last week’s 210.

The Latest Breakthrough Report: Severe Cases Continue to Be Among the Unvaccinated

OHA’s most recent update on COVID-19 breakthrough cases, released today, reported 57,011 cases of COVID-19 during the week of Jan.16 to Jan. 22.  

Of those cases, 40,594, or 71.2% (vs. last week’s 81%) were unvaccinated people and 16,417, or 28.8% (vs. last week’s 19%), were vaccine breakthrough cases.

Ninety-eight breakthrough cases were residents of care facilities, senior living communities or other congregate care settings (up from 53 last week).

To date, there have been 104,088 COVID-19 vaccine breakthrough cases in Oregon, out of a total of 613,121.

The great majority of COVID cases are in unvaccinated people. The report shows that the rate of COVID-19 in unvaccinated people is more than four times as high as fully vaccinated people (down from five times last week), and nearly nine times as high as in fully vaccinated and boosted people. 

Where the vaccines really make a difference is in preventing serious COVID cases.  Nearly all breakthrough cases show no symptoms, or symptoms that are relatively mild.  To date, only 2.9% of the small percentage of vaccine breakthrough cases have been hospitalized (down from 3.2%), and just 0.7% have died (down from 0.8%). Nearly all the COVID patients in ICUs and on ventilators have been unvaccinated.

Serious breakthrough cases are generally found in people who are aged, infirm, and immunocompromised, and who are not as responsive to the vaccine as younger people are.  The average age of vaccinated people who have died remains 81. 

To date, nearly 3 million Oregonians are fully vaccinated.

Note from OHA: As case counts have increased dramatically during the Omicron surge, breakthrough case ascertainment has been unable to keep pace with the volume of reported cases because approximately one-third of recent case vaccination status is unknown. [This may help to explain the fluctuations in percentages that we’ve been seeing over the last few weeks.] OHA is working towards alternative methods to identify vaccination status for all reported cases in a timely manner.

This week’s breakthrough report includes additional information on case vaccination status, most notably whether a breakthrough case was “fully vaccinated” or “fully vaccinated and boosted” at the time of infection. Figure 1 now compares case rates among unvaccinated, fully vaccinated, and fully vaccinated and boosted individuals. In addition, the newly added Table 3 shows the number of cases, hospitalizations and deaths by case vaccination status. 

 

And the Deaths:

Here is information about the 151 additional deaths that have been reported since last Thursday’s newsletter, a big jump from the 46 additional deaths reported last week. 

However, once again this week we’ve seen large data drops from earlier deaths.  Most of this past week’s reported deaths were actually late-reports from December ago and in some cases much earlier.  Only 73 of this past week’s reported deaths occurred in January (still a large increase from last week’s 46).

OHA does have a dashboard that shows week-over-week numbers of deaths by actual date of death, not when the deaths were reported (i.e., they go back and plug those late-reports into the weeks they actually occurred.  You can find that dashboard here. It’s updated once a week.
