Dear Neighbors and Friends,

I hope that you and your loved ones are doing well, staying healthy, and looking out for your neighbors and friends during this past week.

And I want to wish you and your loved ones all the best for a happy and fulfilling Thanksgiving holiday.  We’re fortunate to be spending Thanksgiving with our children and grandchildren, and a few other close family members.  It’s a far cry from last year, when we had to resort to zoom for our Thanksgiving dinner.  This year we’re all vaccinated, and the adults have all had boosters.  Though this pandemic is far from being over, we have so much to be thankful for as a family.

The same really can be said for Oregon as a whole.  As this newsletter will show, Oregon is once again in a much better place disease-wise than most other states.  The various reports, week-over-week numbers, and graphs show steady if slow improvement in our rates of COVID infection, positivity rates, and hospitalizations. The forecast from OHSU is a positive one (though of course with caveats).

Not surprisingly, this report doesn’t include Thursday’s numbers because of the Thanksgiving holiday.  But OHA and OHSU did provide us with their Breakthrough Report and COVID Forecast a day early, so I was able to include them.

We are continuing to see large numbers of COVID deaths here in Oregon, but again many if not most of them are previously-unrecorded deaths that actually occurred during the August and September Delta surge.  They’re not evidence that the pandemic is becoming more virulent.

But they are reminders of just how virulent this disease has been, more so than we previously knew. With their being added into the mix, Oregon’s total number of COVID deaths now surpasses 5,000, every one of them an Oregonian taken from us before their time. 

While it’s appropriate for us to be joyful and appreciative of all that we have today, it’s even more important for us to recognize and thank the many medical workers and public health officials working doggedly on our behalf, despite the insidious way in which this disease has been politicized and the discourse charged with misinformation and misdirected, hateful attacks. 

Please join me in extending heartfelt thanks to all who have been battling this disease day after day and to their families who have been supporting them in this heroic effort.

Special Judicial Panel Rules In Favor of Congressional Redistricting

As expected, the Legislature’s redistricting plan for congressional seats (including the new one coming to Oregon next year) has been upheld by the panel of judges appointed to rule on the challenge brought by a group of former Republican officials.

In brief, the panel found unanimously that the Legislature had followed the constitutional and statutory requirements for redistricting, including the requirement that the maps cannot be “gerrymandered,” i.e., designed to favor one party over another.

The ruling was similar in many respects to the one we received on Monday from the Oregon Supreme Court regarding the legislative redistricting.  However, I found it to be much stronger in its implicit criticism of those who brought the suit forward.  Here’s an example of their critique:

The legal standard they propose ignores Oregon precedent and misapplies the caselaw on which they do rely and the evidence they point to is exaggerated, mischaracterized, or both. Ultimately, the extensive record in this case establishes that, far from being motivated by partisan purpose, the Legislative Assembly drew the enacted map based on public input and neutral criteria—resulting in a fair map that was not drawn for a partisan purpose.

The plaintiffs in the case do have the option of appealing the decision to the Oregon Supreme Court, though given the way that court ruled in the similar cases on Monday, their chances of success aren’t great.  They have until next Monday to do so.

OREGON CORONAVIRUS AND CORONAVIRUS RESPONSE UPDATE

  • New COVID Cases: OHA reports 862 new COVID cases today, vs. 1,160 last Thursday. That’s an average of 749 per day for the six days since the last newsletter (vs. 688 per day the previous week). The cumulative number of cases in Oregon since the beginning of the pandemic is 387,485.
  • Variant COVID Cases:
    • OHA is now showing us not only this week’s variant cases but the growth of the different variants in Oregon over time. You can see in the graph above just how the Delta variant has come to dominate cases here in Oregon.  It is updated every Wednesday.
    • OHA’s Variant Dashboard provides current and new variant case numbers for the state as a whole but also for different parts of the state. It too is updated each Wednesday.
  • Positive Test Results: OHA reported 1,084 positive test results today (vs. 2,114 last Thursday). That’s an average of 929 per day for the 6 days since the last newsletter (vs. 1,341 per day the previous week). The cumulative total of positive test results since the beginning of the pandemic is now 528,125.
  • Total Tests: OHA reported an additional 19,360 tests today (vs. 28,959 last Thursday). That’s an average of 16,016 per day for the 6 days since the last newsletter (vs. 14,990 for the previous week). Our cumulative total of reported tests is 7,903,648. 7,999,746.
  • Positivity Rate: The test positivity ratio for Oregon today was 5.6% (vs. 7.3% last Thursday). That’s an average of 5.4% per day for the 7 days since the last newsletter (it was 6.7% per day for the previous week). 
  • Hospitalization Information:
    • Patients Currently with Confirmed COVID-19: 391 (28 fewer than last newsletter)
    • ICU Patients Confirmed w COVID-19: 97 (7 fewer than last newsletter.)
    • Confirmed COVID-19 Patients Currently on Ventilators: 40 (20 fewer than last newsletter)
    • Available Ventilators: 875 (15 more than last newsletter).
  • Hospital Capacity:
  • Deaths: I’m sorry to report 50 additional COVID deaths today (vs. 13 last Thursday).  That’s an average of 38 per day for the 6 days since the last newsletter (vs. 19 the previous week). The total number of COVID deaths in Oregon is 5,116.
    • ***As noted above, most of the deaths reported this week actually occurred earlier in the pandemic, back when Delta was first surging in Oregon.
  • Vaccinations:
    • Today, OHA reported that 23,407 new doses of COVID-19 vaccinations were added to the state immunization registry on Nov. 23.
    • Of that total, 11,823 were administered that day. There were 2,548 initial doses, 556 second doses and 8,655 third and booster doses. The remaining 11,.584 doses were administered on previous days but were entered into the vaccine registry on Nov.23.
    • The seven-day running average is now 18,737 doses per day (up from 17,986 on Thursday)..
    • Total doses administered: 6,007,214 (up by 142,539 from Thursday)

Weekly County Report: County and Statewide Numbers Stabilize

The latest OHA county report, showing weekly increases/decreases in COVID spread for each county and for the state as a whole, was released on Monday. It shows case counts and rates for the last four weeks, including the week that ended Sunday, November 21.

This report shows that the statewide infection and test positivity rates have pretty much stabilized after three weeks of declines.

Oregon’s 36 counties are showing mixed results, with some seeing more cases and others fewer.  Washington County’s rate went up slightly but still leads the tri-county region (and nearly all other counties) in its infection rate, followed closely by Multnomah County.  Those two counties remain in the “Moderate Risk” category, while nearly all other counties remain at either “High Risk” or “Extreme Risk.” 

 

OHA’s Weekly COVID Data and Outbreaks Reports Released: Fewer COVID Cases, Hospitalizations, and Deaths This Week

OHA’s COVID-19 Weekly Report, released on Wednesday, shows a decrease in daily cases and hospitalizations and an increase in deaths (but again, most of those reported actually occurred two months ago or more).

  • OHA reported 5,698 new cases of COVID-19 during the week of Monday, Nov. 15, through Sunday, Nov. 21. That represents a 2.4% decrease from the previous week, despite a 19% increase in reported tests.
  • There were 133,410 tests for COVID-19 for the week of Nov. 14 through Nov. 20 (up from 111,898 the previous week.
  • The percentage of positive tests declined slightly to 6%, down from 6.5% the previous week.
  • The overall incidence of reported COVID-19 in Oregon varied by county vaccination rate but continues to be less in counties with higher vaccination rates and the lowest in the five counties where the vaccination rates were more than 70%.
  • There were 268 new COVID-19 hospitalizations, down from 356 last week — a 25% drop.
  • There were 214 reported COVID-19-related deaths, up from 183 reported the previous week, bringing the state’s toll for the pandemic to more than 5,000 this week. The past three weeks of data for COVID-19 related deaths also reflect ongoing data reconciliation of COVID-19-related deaths that primarily occurred from May to Aug. 2021, as reported last month.

Today’s COVID-19 Weekly Outbreak Report shows 61 active COVID-19 outbreaks in senior living communities and congregate living settings, a further reduction from the previous week.

Share of Breakthrough COVID Cases Rises Slightly

OHA’s most recent update on COVID-19 breakthrough cases, released yesterday, found that 28.5% of all 5,875 reported cases between Nov. 14 and Nov. 20 occurred in fully vaccinated Oregonians. This is a small increase from last week’s 25.5%.

That means 71.5% of the 5,875 reported COVID-19 cases between Nov. 14 and Nov. 20 occurred in unvaccinated people.

Cases of COVID-19 remain far more common in unvaccinated people. The report shows that the rate of COVID-19 in unvaccinated people is 4 times higher than in vaccinated people.  To date, there have been 43,398 COVID-19 vaccine breakthrough cases in Oregon out of a total of 387,485. 

The number of vaccine breakthrough cases identified in Oregon remains very small when compared to the more than 2.9 million Oregonians who have received at least one dose of a COVID-19 vaccine.

Where the vaccines really make a difference is in preventing serious COVID cases.  Nearly all breakthrough cases show no symptoms, or symptoms that are relatively mild.  To date, only 4.4% of vaccine breakthrough cases have been hospitalized and 1.2% have died.

Serious breakthrough cases are generally found in people who are aged and infirm, and who are not as responsive to the vaccine as younger people are.  The average age of vaccinated people who died was 81. 

With the proliferation of booster shots among the elderly (nearly half of all Oregonians 65 and older have received a booster), we should be seeing a reduction in the death rate among breakthrough cases.

 

New OHSU Forecast Continues to Show Declines in COVID Hospitalizations, Sees Lower Risk of Another Surge in Oregon

The current OHSU forecast report, https://www.ohsu.edu/sites/default/files/2021-11/OHSU-COVID-Forecast-20211124.pdf published November 24, uses data provided by OHA and others that project how fast the virus may spread in the population and provides projections on possible outcomes, including infection rates and impacts on hospital capacity.  The lead author is Dr. Peter Graven, Director of OHSU’s Office of Advanced Analytics.

This forecast sees increasingly lower chance that Oregon will experience another COVID surge this fall and winter, as we’re seeing in Europe and other parts of the U.S. (the Midwest and northeast). They are experiencing what Dr. Graven calls “delayed delta surges.”  Oregon, on the other hand, was already hard hit by the Delta variant in August and September.  As a result of that earlier surge, along with relatively high rates of vaccination statewide in Oregon, only an estimated 19% of the population remains susceptible to COVID-19 (i.e., neither vaccinated nor previously infected). As this rate nears 15%, the forecast predicts, it will become harder for a surge to happen. 

Needless to say, we’re not out of the woods yet, and there are ongoing caveats and concerns:

  • It is increasingly clear that the vaccines provide less protection from a mild case of COVID over time. Keeping the susceptibility rate low as immunity wanes will require boosters or exposure.
  • Oregon will most likely see relatively high caseloads through the winter months but with gradual declines in the number of hospitalized patients.
  • If there’s a lowering of masking requirements or voluntary rates of masking, that could increase the number of cases this winter.
  • Thanksgiving is showing signs of high rates of travel and plans to gather with others. This could generate cases and is consistent with the model expecting further fatigue with prevention efforts.

As for the current state of COVID in Oregon, here are this week’s top-line observations from OHSU:

  • Overall case rates in Oregon continue to decline but at slower rate.
  • The rates among the unvaccinated, however, continues to increase modestly.
  • Relative to other states, Oregon is doing better.  Oregon is now 40th highest in the U.S. in the number of new cases per day. (We were 35th a week ago.)
  • The number of patients needing hospital beds declined to 391 as of Nov. 24, a continued decrease from the week before.
  • All regions in Oregon are showing modest declines in the number of people in the hospital.
  • As of Nov. 22nd, 14% of occupied ICU beds had COVID patients in them statewide, a decrease from the week before.
  • The number of children in Oregon hospitals — three as of Nov. 23 — held steady.
  • Flu cases remain low. The flu season spikes at different times each year, though, so this data may not mean a flu surge won’t happen.

Oregon’s 5,062nd COVID-19 related death is a 69-year-old man from Umatilla County who tested positive Sept. 27 and died Nov. 21 at Providence Portland Medical Center.

Oregon’s 5,063rd COVID-19 related death is a 78-year-old man from Umatilla County who tested positive Oct. 27 and died Nov. 17 at his residence.

Oregon’s 5,064th COVID-19 related death is a 59-year-old woman from Yamhill County who tested positive Nov. 8 and died Nov. 19 at Salem Hospital.
