November 4th COVID-19 Update

Dear Neighbors and Friends,

I hope that you and your loved ones are doing well, staying healthy, and looking out for your neighbors and friends during this past week.

We seem to be entering a tricky period with respect to COVID here in Oregon.  The delta surge continues to abate, which is great.  But there is evidence that Oregonians are beginning to lower their guard when it comes to gathering indoors and becoming more casual about masking while eating in public.  “Normalcy” seems just around the corner, especially now that nearly all children are officially eligible to be vaccinated.  I know I have that feeling myself. 

But as you’ll see in tonight’s newsletter, the pace of decline from the delta surge—though it continues overall—has slowed down.  As a result, we may not find ourselves as far along by the holidays as we had hoped. Oregon, which was once among the very safest of states, is now right in the middle of the pack for new cases.

I’m continuing to receive emails and social media messages from people asking when the state will lift its masking mandates.  I understand the impatience, but I’d say it’s clearly too soon.  As you’ll see in the county report, though we’ve certainly made progress, our COVID numbers are still higher than they’ve been for most of the pandemic. Nearly every Oregon county remains at the High Risk or Extreme Risk level. You'll see in one of the linked news articles below that reversing a mask mandate too soon can have potentially deadly consequences.

The OHSU COVID forecast report tells us that every five COVID hospitalizations will equate to a COVID death (since so many of those hospitalized are unvaccinated and thus highly susceptible). Obviously, we still need to do what we can to keep our rate of infections and hospitalizations down, and thereby our neighbors and family members alive. 

You’ll see in tonight’s newsletter a dramatic increase in the number of reported COVID deaths.  This is obviously tragic and concerning—but taken out of context that statistic will be misleading.  As we were advised last week, OHA is finally making its way through a backlog of COVID deaths that had been slow to come in from the counties and CDC, dating from last spring.  A total of 500 reports need to be processed, and the first hundred or so have just come in.  These reported deaths will be added to their proper place in the OHA mortality graphs.

THURSDAY’S CORONAVIRUS AND CORONAVIRUS RESPONSE UPDATE

  • New COVID Cases: OHA reports 1,491 new COVID cases today (vs. 1,116 last Thursday). That’s an average of 1,050 per day for the 7 days since the last newsletter (vs. 1,091 per day the previous week). The cumulative number of cases in Oregon since the beginning of the pandemic is 371,001.
  • Variant COVID Cases:
    • OHA is now showing us not only this week’s variant cases but the growth of the different variants in Oregon over time. You can see in the graph above just how the Delta variant has come to dominate cases here in Oregon.  It is updated every Wednesday.
    • OHA’s Variant Dashboard provides current and new variant case numbers for the state as a whole but also for different parts of the state. It too is updated each Wednesday.
  • Positive Test Results: OHA reported 1,491 positive test results today (vs. 1,127 a week ago). That’s an average of 1,369 per day for the seven days since the last newsletter (vs. 1,374 per day the previous week). The cumulative total of positive test results since the beginning of the pandemic is now 507,056.
  • Total Tests: OHA reported an additional 22,590 tests today (vs. 15,021 a week ago). That’s an average of 19,427 per day for the seven days since the last newsletter (vs. 18,114 for the previous week). Our cumulative total of reported tests is 7,644,894.
  • Positivity Rate: The test positivity ratio for Oregon today was 6.6% (vs. 7.5% a week ago). That’s an average of 7.1% per day for the seven days since the last newsletter (it was 7.5% per day for the previous week). 
  • Hospitalization Information:
    • Patients Currently with Confirmed COVID-19: 532 (14 more than last newsletter)
    • ICU Patients Confirmed w COVID-19: 123 (the same as last newsletter.)
    • Confirmed COVID-19 Patients Currently on Ventilators: 62 (7 more than last newsletter)
    • Available Ventilators: 856 (16 fewer than last newsletter).
  • Hospital Capacity:
  • Deaths: I’m sorry to report 74 additional COVID deaths today (vs. 24 last Thursday).  That’s an average of 26 per day for the week since the last newsletter (vs. 12 per day the previous week).  The total number of COVID deaths in Oregon is 4,543.

***Note: Most of today’s 74 and yesterday’s 64 were late reports from last spring.

  • Vaccinations:
    • Today, OHA reported that 18,096 new doses of COVID-19 vaccinations were added to the state immunization registry on Nov. 3. Of that total, 1,126 were initial doses, 840 were second doses and 9,798 were third doses and booster doses. The remaining 6,270 were administered on previous days but were entered into the vaccine registry on Nov. 3.
      • 3,323,667 Pfizer/Comimaty vaccines
      • 2,039,930 Moderna vaccines
      • 239,623 Johnson & Johnson vaccines
    • Today’s seven-day running average is 17,085 doses per day (an increase from last week’s 9,309, much of it due to boosters).
    • Total Oregonians vaccinated so far: 2,825,299 (up by14,569 from last week)
      • 2,580,142 now fully vaccinated with two doses (up by 38,732)
    • 68.6% of all Oregonians have received at least one dose (20th in the nation)
      • 80.5% of those 18 and older
      • 79.1% of those 12 and older
      • 93.4% of those 65 and older
    • 63.1% of all Oregonians are now fully vaccinated
      • 74.2% of those 18 and older
      • 72.7% of those 12 and older
      • 87.7% of those 65 and older
    • 13.1% of all Oregonians have received a booster shot
      • 14,0% of those 18 and older
      • 21.8% of those 50 and older
      • 31.8% of those 65 and older

OHA Releases Latest County Metrics: Upward Movement Again

The latest OHA county report, showing weekly increases/decreaIses in COVID spread for each county and for the state as a whole, was released on Monday. It shows the latest case counts, including the week that ended Sunday, October 31.

The report shows our statewide infection rate has risen slightly (breaking a pattern of reductions that has continued for more than six weeks), while test positivity rates remain the same as last week: 7.4%.  Most individual counties, including Multnomah County, rose last week, including both in infection rates and positivity rates.

Multnomah and Clackamas Counties now remain firmly in the “High Risk” category, while Washington County rose only slightly and remains close to the “Moderate Risk” threshold. (Unlike Multnomah and Clackamas, which saw their test positivity rates increase, Washington’s test positivity rates continued to decline last week.)

Despite these increases, the state as a whole (and nearly every county) remain lower than they  were two or three weeks ago.  The overall long-term trend still seems to be downward, confirming the ongoing retreat of the delta surge of August and September.  That’s in line with what the OHSU and OHA forecasts have been predicting.

 

Weekly COVID Data and Outbreaks Reports Released: More COVID Cases, Fewer Hospitalizations and Deaths

Oregon Health Authority’s COVID-19 Weekly Report, released yesterday, shows an increase in daily cases and decreases in hospitalizations and deaths.

  • OHA reported 7,877 new cases of COVID-19 during the week of Monday, Oct. 25 through Sunday, Oct. 31. That represents a 2.2% increase from the previous week and the first increase after eight consecutive weeks of declining counts.
  • The incidence of reported COVID-19 cases continues to be higher in Oregon counties with population vaccination rates less than 50%.
  • There were 313 new COVID-19 hospitalizations, down from 415 last week, marking the lowest number of hospitalizations since the week of July 26 – Aug. 1, 2021. (!!!)
  • There were 82 reported COVID-19 related deaths, down from 110 reported the previous week.
  • There were 130,325 tests for COVID-19 for the week of Oct. 24 through Oct. 30, down from the previous week’s 137,537.
  • The percentage of positive tests was 7.4%, unchanged from the previous week.

Yesterday’s COVID-19 Weekly Outbreak Report shows 87 active COVID-19 outbreaks in senior living communities and congregate living settings. 

This continues the steady reduction we’ve been seeing over the last six weeks. We’re now back down to pre-Delta counts in congregate care settings.

Share of Breakthrough COVID Cases Goes Down, Unvaccinated Cases Up

OHA’s most recent update on COVID-19 breakthrough cases, released today, found that 2,025, or 24.6% of the 8,239 reported COVID-19 cases between Oct. 24 and Oct. 30 occurred in fully-vaccinated people (i.e., breakthrough cases).  This is a reduction from the 27.9% that we saw last week.

Cases of COVID-19 remain far more common in unvaccinated people. To date, there have been 37,539 COVID-19 vaccine breakthrough cases in Oregon (approximately 10% of the total 371,001 COVID cases in Oregon), and an extremely tiny fraction of the more than 2.8 million Oregonians who have received at least one dose of a COVID-19 vaccine.

The report shows that the rate of COVID-19 in unvaccinated people is currently four times higher than in vaccinated people.

Nearly all of the breakthrough cases involve mild or no COVID symptoms.  To date, only 4.4% of all vaccine breakthrough cases have been hospitalized and only 1% have died. The average age of vaccinated people who died was 80.

 

New OHSU Forecast Shows a Slowdown in the Decline of Hospitalizations

The current OHSU forecast report, OHSU Forecast Report, published this afternoon, uses data provided by OHA and others that project how fast the virus may spread in the population and provides projections on possible outcomes, including infection rates and impacts on hospital capacity.

Here are this week’s top-line observations:

  • The forecast expects hospitalizations to continue falling, but at much slower pace than right after the peak of the delta surge.
  • The updated forecast shows that the number of hospitalized Oregonians will continue its slow descent through the holiday season. The updated forecast shows hospitalizations will fall to about 200 by the new year – down from the pandemic’s peak of 1,178 on Sept. 1, 2021.
  • There is still a risk that the flattening could lead to an increase in hospitalizations if fatigue with safety measures hits above expected levels.
  • The number of patients needing hospital beds declined to 527 as of Nov. 3, a 27-bed decrease from the week before.
  • Region 7 continues to have a higher level of patients in hospitals than other parts of Oregon.
  • People under age 20 are estimated to make up 49% of the susceptible population in Oregon.
  • The expansion of vaccine eligibility to ages 5-11 is expected to decrease the susceptible population by 12%, likely over several months.
  • As of Nov. 3, 18% of occupied ICU beds had COVID patients in them statewide, a decrease from last week.
  • Nine children were in Oregon hospitals as of Nov. 2, holding at a steady level.
  • Only small numbers of flu cases are being detected each week in Oregon.
  • The masking wearing rate amongst unvaccinated increased to 58% in last week. The rate amongst vaccinated remained steady at 90%.
  • Oregon ranks fifth in the U.S. in the percentage of unvaccinated people who indicate wearing masks all or most of the time in public.
  • Indoor restaurant and bar visits increased slightly for the vaccinated to 30%, nearly equivalent to the rate for unvaccinated.
  • Vaccinated and Unvaccinated had small increases in large indoor events (15% and 25%, respectively).
  • Time with others indoors is back to pre-surge levels for both the vaccinated and unvaccinated.
  • The recent week of data by date of death showed an increase. This is expected to decrease and follow the hospitalization pattern.
  • Assumptions to project deaths from current infections/hospitalizations: 1) Deaths lag infections by an average of 21 days. 2) There are 5.0 COVID hospitalizations per death from COVID.

Oregon’s 4,449th COVID-19 related death is a 79-year-old woman from Jackson County who tested positive on Oct. 15 and died on Oct. 28 at her residence.

Oregon’s 4,450th COVID-19 related death is a 73-year-old man from Jackson County who tested positive on Oct. 14 and died on Nov. 1 at Providence Medford Medical Center.

Oregon’s 4,451st COVID-19 related death is a 33-year-old woman from Jackson County who tested positive on Oct. 2 and died on Oct. 31 at Asante Rogue Regional Medical Center.

Oregon’s 4,452nd COVID-19 related death is a 52-year-old man from Jackson County who tested positive on Oct. 1 and died on Nov. 1 at Providence Medford Medical Center.

Oregon’s 4,453rd COVID-19 related death is a 59-year-old man from Douglas County who tested positive on Sept. 1 and died on Nov. 1 at Mercy Medical Center.

Oregon’s 4,454th COVID-19 related death is a 79-year-old man from Deschutes County who tested positive on Oct. 6 and died on Oct. 11 at St. Charles Bend Hospital.

Oregon’s 4,455th COVID-19 related death is a 76-year-old man from Deschutes County who tested positive on Oct. 5 and died on Oct. 19 at St. Charles Bend Hospital.

Oregon’s 4,456th COVID-19 related death is a 66-year-old woman from Deschutes County who tested positive on Oct. 4 and died on Oct. 9 at St. Charles Bend Hospital.

Oregon’s 4,457th COVID-19 related death is a 74-year-old man from Deschutes County who tested positive on Oct. 2 and died on Oct. 4 at St. Charles Bend Hospital.

Oregon’s 4,458th COVID-19 related death is a 66-year-old woman from Baker County who tested positive on Oct. 22 and died on Oct. 28 at her residence.

Oregon’s 4,459th COVID-19 related death is a 71-year-old man from Washington County who tested positive on Oct. 18 and died on Oct. 31 at OHSU Hillsboro Medical Center.

Oregon’s 4,460th COVID-19 related death is a 51-year-old man from Washington County who tested positive on Oct. 21 and died on Nov. 1 at OHSU Hillsboro Medical Center.

Oregon’s 4,461st COVID-19 related death is a 72-year-old man from Polk County who tested positive on Oct. 30 and died on Nov. 2 at Salem Hospital.

Oregon’s 4,464th COVID-19 related death is a 61-year-old woman from Marion County who tested positive on Oct. 19 and died on Oct. 28 at Salem Hospital.

Oregon’s 4,465th COVID-19 related death is a 100-year-old woman from Marion County who tested positive on Oct. 15 and died on Oct. 22 at her residence.

Oregon’s 4,466th COVID-19 related death is a 101-year-old man from Marion County who tested positive on Oct. 14 and died on Oct. 20 at his residence.
