Organizations interested in becoming a health-related social needs (HRSN) provider can start preparing for the 2025 Community Capacity Building Funds (CCBF) grant application process.
What are Community Capacity Building Funds?
Oregon Health Authority (OHA) provides coordinated care organizations (CCOs) with Community Capacity Building Funds (CCBF). CCOs administer this grant program. They award CCBF grants to community partners to help them become health-related social needs (HRSN) providers.
Health-related social needs (HRSNs) are social and economic needs that affect your ability to maintain health and well-being. HRSN benefits include:
- Housing
- Nutrition
- Outreach and engagement (help getting other benefits)

How to apply
Organizations that would like to apply for CCBF grants need to contact their local CCO to apply.
On March 1 CCOs will post specific details on their CCBF web pages about:
- Funding amounts
- Funding goals and strategies
- High-level HRSN provider contract requirements
