Community capacity building fund grants available from coordinated care organizations for community partners looking to deliver new benefits to Oregon Health Plan members
Organizations interested in providing health-related social needs services are encouraged to apply for funds starting March 1
Portland, Ore. – For community organizations and partners interested in delivering new health-related social needs (HRSN) benefits to eligible Oregon Health Plan (OHP) members, applications for community capacity building fund (CCBF) grants will open March 1 through May 31.
Oregon is expanding benefits to eligible OHP members through services called health-related social needs (HRSNs), which can help people with social and economic needs that are essential to maintain health and well-being. This includes climate resources (becoming available March 2024), housing benefits (becoming available November 2024) and nutrition services (becoming available January 2025).
Community-based organizations (CBOs) and partners will be integral to delivering these new services and may be able to receive funding to help build their capacity to provide HRSN services.
Organizations that intend to serve eligible OHP members and have strong community relationships, specifically the ability to provide HRSN services in a way that is culturally responsive and ensures meaningful access to language services, are encouraged to apply for CCBF. Funds can be used for technology, workforce development, development of business practices or outreach and education related to the provision of HRSN services.
Organizations eligible for CCBF can be in the following categories: CBOs, social-services agencies, traditional health care workers, child welfare providers, HRSN network managers, government agencies or providers that focus on housing, nutrition, climate, case management, outreach and engagement.
Coordinated care organizations (CCOs) will administer the grants process, except for those funds reserved for Tribal Governments. CCOs will be responsible for conducting outreach, receiving and reviewing applications and awarding funding. Organizations can apply for CCBF through the CCO in their service area. Organizations may apply to more than one CCO as needed, as long as the funding requests are not duplicative.

Notices to awardees will go out in July and September. A list of statewide awardees will be available on the OHA web site. CCOs will disperse funds August to October 2024.
CCBF grants and HRSN services are part of Oregon’s 2022 - 2027 1115 Medicaid waiver, which expands coverage to eligible OHP members and delivers additional federal funding to the state. Medicaid waivers allow states the flexibility to test new ways to deliver and pay for Medicaid benefits, with approval from the Centers for Medicare and Medicaid (CMS). Oregon’s 1115 Medicaid waiver includes several first-in-the-nation benefits for OHP members.
