Dear Superintendents,
This message provides information about the three surveys districts are required to make available during the 2024-25 school year.

*Charter schools are not required to make the SHS and SEED survey available to students, but they are highly encouraged to do so.
Key Dates
Survey Windows:

October – December 2024: Student Health Survey (SHS)

February 4 – June 13, 2025: Student Educational Equity Development (SEED) Survey

Action Required

Student Health Survey - School districts must:
- Make the SHS available to students in grades 6, 8, 11 during a two-week survey window, selected by the school districts, during the months of October through December, 2024;
- Designate a District SHS coordinator to facilitate the survey by completing a Superintendent Approval Form from the SHS vendor, Bach-Harrison. Bach-Harrison will train and provide guidance and technical assistance to the District Coordinator for SHS implementation;
- Provide notice of the survey to the parents or guardians of the students to whom the survey will be administered (option to use a notification template provided by SHS vendor Bach-Harrison);
- Provide the opportunity for parents or guardians to review the survey before it is administered;
- Allow parents/guardians and students to decline participation, verbally or in writing before a student begins the survey.
- Bach-Harrison will guide the District Coordinator identified on the Superintendent Approval Form through the survey administration process and provide all needed materials and technical assistance.

Student Educational Equity Development Survey - School districts must:
- Make the SEED survey available to students (grades 3-11) during survey window: February 4, 2025 - June 13, 2025
- Ensure that the District Test Coordinator (DTC) or their designate will coordinate the administration of the SEED survey, including the required training of School Test Coordinators (STCs) and Test Administrators (TAs)
- Provide notice of the survey to the parents or guardians of the students to whom the survey will be administered;
- Provide the opportunity for parents or guardians to review the survey before it is administered;
- Allow parents/guardians and students to decline participation, verbally or in writing;

Elevating Voices in Education (EVE) Workforce Survey - School districts and charter schools must:
- Ensure licensed and classified staff are provided access to the survey (via web-based link) and have an opportunity to participate during the survey window: February 4 - March 21, 2025
- ODE recommends assigning a district administrator (HR Director, Superintendent, etc.) to coordinate administration of the survey.
- Encourage staff to participate in the survey.
- More information will be provided to Superintendents in Fall 2024.
Background Information
To honor the importance and necessity of incorporating student and staff voices into the continuous improvement process for Oregon’s public education systems, in the 2023 session, the Oregon Legislature passed:

House Bill 2656, which requires school districts to make available to students the Student Health Survey (SHS) and Student Educational Equity Development (SEED) Survey annually. Prior to this legislation, these surveys were optional.
- According to HB 2656, parents must be able to review the surveys and decline their students’ participation. Students must also be able to decline to participate. While the SHS and SEED Survey are both required because of the same legislation, the two surveys address different topics.

Senate Bill 283, which requires districts and charter schools to encourage and ensure licensed and classified staff have the opportunity to participate in the Elevating Voices in Education (EVE) Workforce Survey, an annual survey on working experiences administered by ODE. The EVE Workforce Survey replaces previous surveys such as the Teaching, Empowering, Leading and Learning (TELL) survey and the Oregon Statewide Educator Survey (OSES), and expands to include all licensed and classified staff.
