CACFP Training Survey

Submit by July 31, 2024

ODE CNP wants your feedback on CACFP trainings!

ODE CNP is working on new trainings for Oregon CACFP sponsors and we want your help to create trainings that will better meet your needs. ODE CNP is planning to update the CACFP Policy and Procedure Manual Training Videos and create new trainings for annual training 2025 (yes, already!). 

Tell us what you want to see for 2025.  Your honest and anonymous feedback will help us to create better trainings for all Oregon sponsors. 

To help us best utilize our resources and better understand the needs of our Sponsors, we encourage ALL CACFP Sponsors to respond to this 5-10-minute survey.  Respondents do not need to complete all questions, however more details will help us develop better trainings for everyone.

To file a program discrimination complaint, a Complainant should complete a USDA Program Discrimination Complaint Form which can be obtained online at: Filing a Program Complaint as a USDA Customer webpage, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 Complaint form or letter must be submitted to USDA by:
