Section 504 Sample Forms Now Available

Section 504 sample forms are here!

The Oregon Department of Education (ODE) is pleased to announce the publication of new Section 504 sample forms on the ODE website. The sample forms can be found on the ODE Section 504 Webpage

Please note that sample forms provided by ODE are intended to assist school districts in implementing Section 504. The use of the sample forms is not mandatory, and school districts and programs may choose to develop their own forms or procedures that align with the requirements of federal and state statutes, including Section 504 of the Rehabilitation Act of 1973. ODE encourages school districts to consult with legal counsel, as necessary.

ODE will rollout the translation of family-facing sample forms and they will become available on the Section 504 webpage.
