Dear School and District Leaders,

Oregon Health Authority (OHA) is offering portable high-efficiency particulate air (HEPA) filtration equipment and cleaning supplies to all K-12 schools in Oregon. In December 2022, OHA sent out a survey regarding HEPA filtration units and cleaning supplies, which many of you completed. Equipment and supplies may now be ordered using the link below.

Please consider the following when completing the order form:

Responses may be submitted at the school or district level.
Please submit only one response per school (i.e., do not order supplies for the same school at both the school and district levels).
Please include shipping address and delivery instructions on the order form. Large requests may be received via freight.

Please complete this order form for HEPA air filtration units or cleaning supplies by April 28, 2023. For questions or assistance, please email schooltesting.covid@odhsoha.oregon.gov.
