Accommodations Form and New Requirements for House Bill 4140

 

As a result of requirements from House Bill 4140 (2020), the Oregon Department of Education (ODE) has developed and posted the Traumatic Brain Injury Accommodations Form Word document and PDF fillable form. HB 4140 requires that ODE make this form available to public education programs in Oregon.

School districts and education programs also have new requirements under HB 4140. According to the law, public school programs must make this form available to educators, staff, students, and parents or guardians as soon as practicable. See section 4, which reads: (4)(a) A public education program must make the form developed under this section available when (A) The public education program receives notice that a student has been diagnosed with a concussion or other brain injury; or (B) Requested by an educator, a program employee, a student, a parent or a guardian.”

This form will be available on ODE’s Student Health Conditions page, Traumatic Brain Injury page, and through University of Oregon’s Center on Brain Injury Research and Training (CBIRT). Please visit those pages in coming months for future guidance and supports, including an upcoming webinar.
