New Opt-in Form For Indoor K-12 Athletics

Dear School and District Leaders,

Governor Brown and the Oregon Health Authority shared updated guidance today for indoor K-12 athletics and other indoor recreation and indoor fitness activities.

Mandatory Opt-in Form
The guidance requires school districts and private school governing bodies in extreme or high-risk counties to opt-in to indoor K-12 full-contact sports.

Please follow these steps for submitting the contact sports form:

Step 1: Download and complete this Contact Sports Form template.

Step 2: Upload and post your completed form on your school or district website* that is publicly accessible.

Step 3: Have the link where your form is posted ready to share with ODE. You should choose a link that will stay current if or when you update the form.

Step 4: Complete this simple ODE form to provide the link to where your posted contact sports form lives on your own school or district website.

* Note: If you don’t have a school or district website, you can email the completed form to ODECOVID19@ode.state.or.us .

Your form must be complete, posted, and submitted to ODE prior to beginning indoor full contact sports when in an extreme or high-risk county.
