The OHP Plus (BMM, BMH, BMD) and CWM Plus (CWX) benefit packages cover routine eye exams (CPT codes 92002-92014, with primary diagnosis Z01.00) for any eye condition as follows:
- Once every 24 months for adults age 21 or older with the OHP Plus (BMM, BMH, and BMD) benefit package.
- When clinically appropriate for OHP Plus members under age 21 and CWM plus (CWX) members. Corrective lenses are only covered if the member is under age 21, pregnant, or has a condition related to aphakia or keratoconus:
- Keratoconus, unspecified
- Keratoconus, stable condition
- Keratoconus, acute hydrops
- Aphakia
- Congenital Aphakia (congenital absence of lens)
- Absence of eye lens(es), such as following cataract extraction

Are you an Oregon Health Plan (OHP) Open Card member? Open card is also known as Fee-for-Service or FFS. Do you work with OHP Open Card members? Your experiences are valuable to OHA. We are seeking members for our advisory panel. Panel members will help OHA make policy and program decisions based on member needs.
Who are we looking for?
At least 15 people from across the state who represent different types of Oregonians. Members will be one or more of the groups below:
- Past or present OHP Open Card members (or direct family of members)
- Community Health Workers
- Certified OHP assisters
- OHP member advocates

When and where are meetings?
All meetings will be held virtually.
Fall 2023 – December 31, 2023
- One orientation meeting
- Two 5-hour meetings

- One 1-hour meeting per month
- Extra time as needed to review materials
Will I be paid for my work?
Yes! Committee members are eligible for $157 for each day of work.
Can I get help applying?
Yes! You can get free help or information in a format that works best for you, including Braille, audio, large print, other languages, and other formats. You can also ask for an interpreter.
