County Amendments to Death Records
Past trainings, newsletter articles and Instructional Memo 2016-06 had additional restrictions on changes to marital status, spouse name and informant that are no longer applicable to county amendments when the affidavit indicates that the reason for correction is due to a clerical error. County vital records staff may now amend these items while the original record is still in their possession if they have a legally sufficient affidavit that is signed by the funeral director and has “1” for clerical error in the Reason # column on the Affidavit to Correct a Death Record.
 If a change to marital status, spouse name or informant is requested, the affidavit must a) accompany the death record when it is delivered to the county for initial filing -OR- b) clearly identify that it is due to a clerical error. Otherwise, do not amend the record unless it is a minor spelling error. If the change is not a minor spelling error or clerical item, please refer it to the State Vital Records Office.

Welcome, James Kracker
CHS is excited to welcome James Kracker to the Partner Services team. His responsibilities include coordinating logistics and materials for all the training conducted by the team to external partners throughout the state. James will also be coordinating logistics for triennial reviews, maintaining training plans, tracking training requirements and certifications, writing and reviewing newsletter articles, generating monthly reports to be shared with our partners, and providing administrative support to the team.

Hi, I’m James (he/him) and I’m thrilled to join the team! I grew up in the mountains of South-Central Alaska and fell in love with Oregon 20 years ago when I arrived in Eugene to pursue my Master of Fine Arts. I’ve been passionate about writing since I was old enough to hold a pencil, and editing, tutoring, and teaching soon followed. In my spare time, I’m likely to be lost in the woods or found doodling cartoons at the drawing table.

Farewell, Gretel Bohn
Congratulations to Gretel Bohn, member of the CHS Registration Unit, on beginning her new career as an edible landscape designer and regenerative agriculturist.
Gretel shared the following about her new adventure:
I will be at a local farm following my passion for regenerative agriculture, nursery and fruit arborist work! I will be helping families transform their yards into permaculture food-forest guilds, tending fruit orchards, and working with a grant given by the Oregon Food Bank on a community project to address local food insecurity. I will miss working with you all – thank you for having me.

Farewell, Nick Bickford
Congratulations are also in order for Nick Bickford, Amendments Specialist, on his new position as a Death Benefit Support Assistant with Oregon Public Employees Retirement System (PERS).
We thank Nick for his collaboration and contributions to the Amendments team, work with funeral homes and partners, and his commitment to assisting and serving all Oregonians. He will be greatly missed!
