Help improve PEBB wellness programs!
Dear PEBB Member,
In the next few days, you may receive an email asking you to take a short survey about your health and quality of life. PEBB is working with Providence’s Center for Outcomes Research & Education (CORE) to understand the impact of PEBB’s wellness programs, including the Health Engagement Model and Healthy Team Healthy U. The email will include a unique link to the survey.
The survey will cover topics such as your experience with PEBB wellness programs (if any), as well as your health and well-being. The survey is web-based and will take 15-20 minutes to complete, and all responses are confidential and reported in aggregate. Your participation is voluntary, and your responses will not affect your employment or any benefits from PEBB wellness programs. By completing the survey, you will be entered into a drawing to receive one of 25 Visa gift cards worth $20.
