SDE: Dynamic Learning Maps First Contact Survey

Dear Superintendents and District Testing Coordinators,

We are requesting that all special education teachers in Oklahoma who have students assessed with the OAAP Portfolio fill out a” First Contact Survey” for each of their students. The survey was created by  the Dynamic Learning Maps (DLM) consortia, and the information will be used to inform assessment decisions as well as identify needed personal information about  students. 

Thank you for sharing the attached letter with the appropriate teachers in your district.
