This is the application for districts who serve less than 10 students or operate with abbreviated hours/days. Please remember this document must be approved by your school board and the state school board.  Don't delay getting this completed this summer.

Teaching Out of Certification form

This is the form for certified teachers (not adjunct teachers)  who need additional certifications for the alternative education program. Once approved by your Alt Ed specialist these forms need to be uploaded into digital criteria folder #4.

Checklist - FY24

This form will assist you with the completion of your 16 Criteria digital folders, which are due October 1, 2023.  Use this checklist to inform what you need to put in each folder.  Remember to upload .PDF and .JPG files only. 

1. Form - At Risk Indicator - this form must be completed for all students enrolled in the Alt Ed program in FY24.  Once completed all forms need to be uploaded into digital criteria folder #3 High School Support Indicator 
2. Student Led Conferences - this is a great form for your program to modify to use as an individual learning plan for each of your students.  The goal of this document is for each student to have a written plan for graduating and post-graduation next steps.  Students should be able to talk confidently about their plan, where they are in the process, and what happens next. Sample of Student Led Conferences

Cooperative MOU

This is a sample/draft MOU for districts involved in a co-op. Our department worked this up from many MOUs across the state.  We wanted to provide an example of the various sections from MOUs other districts are using. Please feel free to retype it and customize it for your district.  You may delete or add sections as your district deems appropriate.  We encourage you to provide your edited MOU to your district's legal department before sending to your Cooperative Districts.  Sample MOU 

Authorization to Pay

This document needs to be completed by all sending schools to an alternative education co-op.  Once completed a copy needs to be sent to the co-op's LEA and uploaded into the sending district's alternative education implementation plan on single sign-on and digital criteria folder #1.

We know this is a lot of information at such a busy time of year, but we wanted to get it all out before the end of the year. We suggest you save this newsletter in a folder so you have it throughout the year.
