Disability Determination Services Teacher Questionnaire Form

A new document will be available in OK EdPlan on 9/27/2018. It is a teacher questionnaire required of Disability Determination Services (DDS) for the purposes of verification of Social Security benefits. If you are contacted by a representative from DDS to complete a "DDS Teacher Questionnaire: SSA 5665," please download the document on your computer, complete the required information and upload the information to the student's "documents" tab as an external document. Reach out to the DDS representative to acknowledge completion. This form is not used unless requested.


School Based Health Services

School Based Services Participation Agreement is now available through the Grants Management System. All districts choosing to use this agreement may begin completing their online application. Follow these steps for application completion.

  1. Go to "GMS Access/ Select" and scroll to available applications under IDEA.
  2. Click "Create" on the line for "School Based Health Part". Once you have created the application it will appear under the applications.
  3. Please agree to all assurances, upload the signature page, and submit.

Critical: Child Count Collection Notice

Please note that the FY2019 Child Count is October 1,2018. All errors and missing data in a student's record must be resolved for that student to be counted in child count, and every student must have an active IEP and an eligibility determination. Please review your Child Count Report in Advanced Reporting for an accurate list of students in your district.

If someone from your district did not attend the in-person training or webinar, please review the slides here. The training reviews all possible errors and how to resolve them. It also covers how to open the reports and the certification process.

Because of the data transfer issues from the Wave to EdPlan, we are instituting a formal child count appeal process. More information will be available soon on how to submit an appeal and when it may be appropriate.

AT Support Team Training: Register Now

The first of ABLE Tech’s three fall AT Support Team workshops at Tulsa Tech, Owasso Campus, is just a few days away. Workshops are filling fast, so register today! Registration will be opening soon for our spring workshops at Cameron University in Lawton. AT trainings are provided at no cost to Oklahoma educators and related service providers. Continuing Education Units (CEUs) will be offered for OTs, PTs, and SLPs. See the workshop descriptions below and click here for training details and registration links.

AT Assessment Part 1- Consideration
Choose one:

  • Spring Session: January 17, 2019, Cameron University, Lawton

Description: Participants will learn the process for Consideration of Assistive Technology (AT) for all students with disabilities, including legal requirements related to AT Consideration, and hands-on learning to help educators discover potential solutions available to help meet student’s IEP goals.
 
AT Assessment Part 2 – Gathering Information and Trialing Devices
Choose one:

  • Fall Session: October 11, 2018, Owasso Tech Center
  • Spring Session: February 14, 2019, Cameron University, Lawton

Description: Participants will learn the steps involved in conducting an Assistive Technology (AT) Assessment, and the procedure for borrowing AT to try with students. Educators will try their hand at making low-tech AT to help students with classroom tasks.

AT Assessment Part 3 – AT in the IEP and Implementation
Choose one:

  • Fall Session: November 8, 2018, Owasso Tech Center
  • Spring Session: March 14, 2019, Cameron University, Lawton

Description: Participants will learn how to write AT in the IEP, summarize AT Implementation process, compare and contrast AT across disabilities, and identify funding resources for acquisition of AT. Afternoon breakout sessions will offer educators opportunities to see and try devices in the categories of their choosing.


Additional training resources:

  • AEM/Bookshare Workshops: ABLE Tech provides “AEM and AT for Reading” workshops at no cost for districts upon request. Learn how to deliver Accessible Educational Materials (AEM), including digital text from Bookshare, for students in your district. All you need to do is gather participants and provide the training space. Distance-learning is also available through Zoom meetings.

    Differentiated Monitoring Process

    Last Year, OSDE-SES adopted a results-based accountability approach into its data monitoring and reporting processes. Performance measures were permanently incorporated into the determination, and a risk assessment was added. Both are now used to determine differentiated levels of support for all districts across Oklahoma. How much do you know about this process? Whether or not you attended the recent training in Oklahoma City, Sapulpa, McAlester, Lawton or Woodward, we want your feedback on this process as implemented last year.
