A new document will be available in OK EdPlan on 9/27/2018. It is a teacher questionnaire required of Disability Determination Services (DDS) for the purposes of verification of Social Security benefits. If you are contacted by a representative from DDS to complete a "DDS Teacher Questionnaire: SSA 5665," please download the document on your computer, complete the required information and upload the information to the student's "documents" tab as an external document. Reach out to the DDS representative to acknowledge completion. This form is not used unless requested.
School Based Services Participation Agreement is now available through the Grants Management System. All districts choosing to use this agreement may begin completing their online application. Follow these steps for application completion.
- Go to "GMS Access/ Select" and scroll to available applications under IDEA.
- Click "Create" on the line for "School Based Health Part". Once you have created the application it will appear under the applications.
- Please agree to all assurances, upload the signature page, and submit.
Please note that the FY2019 Child Count is October 1,2018. All errors and missing data in a student's record must be resolved for that student to be counted in child count, and every student must have an active IEP and an eligibility determination. Please review your Child Count Report in Advanced Reporting for an accurate list of students in your district.
If someone from your district did not attend the in-person training or webinar, please review the slides here. The training reviews all possible errors and how to resolve them. It also covers how to open the reports and the certification process.
Because of the data transfer issues from the Wave to EdPlan, we are instituting a formal child count appeal process. More information will be available soon on how to submit an appeal and when it may be appropriate.

AEM/Bookshare Workshops: ABLE Tech provides “AEM and AT
for Reading” workshops at no cost for districts upon request. Learn how to
deliver Accessible Educational Materials (AEM), including digital text
from Bookshare, for students in your district. All you need to do is
gather participants and provide the training space. Distance-learning is
also available through Zoom meetings.

Performance measures were permanently incorporated into the determination, and a risk assessment was added.
