*New Form* - Employer Certification of Separation of Service
Each time you have an employee separate service who does not immediately retire, we ask you now complete the Employer Certification of Separation of Service form. This form allows us to capture certain dates and unused sick leave information. This is important information if the member should later qualify for disability retirement, start a vested retirement benefit or request a withdrawal of their employee contributions.
The digital form is completed online using Adobe Sign. The digital form is easy to complete and efficiently adds the information to the member’s file. After digitally signing the form, check your inbox for an email to confirm your signature.
This form is available on the Employers > OPERS Forms page under the “Certifications and Enrollment” heading.
Note: State agencies on the WorkDay payroll system do not need to complete this form. We currently receive this information through WorkDay.
Elected Officials and Unused Sick Leave
The Employer Certification of Separation Form should also be completed for employees who move from a non-elected position to an elected position, even if the position is with the same county. This form will track the amount of unused sick leave the employee had before becoming an elected official. Elected official positions do not accrue or use sick leave. However, unused sick leave balances from non-elected positions while participating in OPERS may be used toward their future retirement benefit calculation. Forms are now available for Deputy Sheriffs and Jailers to Purchase Prior Service
House Bill 1068 allows county deputy sheriffs and jailers to purchase prior service as a county deputy sheriff or jailer at actuarial cost. Service purchased will count toward the 20-year retirement eligibility rule. Once purchased, this service will be calculated using the higher 2.5% computation factor.
As the retirement coordinator, you will need to complete the Employer Certification of Prior Service form for your current or past employees who would like to purchase this service. This form is available on the Employers > OPERS Forms page under the “Certifications and Enrollment” heading.
Your employees can start this purchase by completing the Member Request for Service Credit purchase on our forms webpage.
