Oklahoma Universal Service Fund - No changes to required forms for Funding Year 2022

In compliance with 17 O.S. §§ 139.106(D)(4), 139.109.1(F)(2)(a), and OAC 165:59-3-61, the Oklahoma Universal Service Fund (“OUSF”) Administrator determined that the forms required to request funding from the OUSF will remain unchanged for Funding Year 2022, which begins on July 1, 2022. Therefore, for all requests filed after this date, please continue using the forms already posted to the Commission’s website.
