Provider Files and Renewals

To align with EDI and SoonerSelect standards, OHCA has added “9999” to the ZIP+4 field for all providers where the ZIP+4 was missing. No changes were made to provider files if a ZIP+4 code was already on file. If OHCA made changes to your file, please include “9999” on the ZIP code for the billing, rendering and/or ordering provider(s) as needed when submitting claims to OHCA or our SoonerSelect partners. ZIP+4 codes must be unique to each provider service location. If claims are rejected due to ZIP code issues or if you believe your location may have a duplicate “9999” value, please check your provider portal or the OHCA Provider Directory for your assigned ZIP+4. You may also contact Provider Enrollment to confirm the correct ZIP+4 on file or request an update.

OHCA will be extending all ITU contracts that expired on July 31, 2025, to Oct. 31, 2025.

For the following provider types, 75-day renewals started July 17, and contracts expire Sept. 30, 2025.

  • Dentist
  • Genetic counselor
  • Independent diagnostic testing facility
  • Independent lab
  • Lactation consultant
  • Maternal and infant health LCSW
  • Room and board

For the following provider types, 75-day renewals start Aug. 17, and contracts expire Oct. 31, 2025.

  • Inpatient psychiatric facilities

Please be sure the contact information, including email address, is current for all individual and facility/group provider files so you can quickly receive important contract information.


SoonerCare Benefits for Patients Who Smoke

According to the American Lung Association, less than 2% of all patients considered at high risk for lung cancer are screened.
Lung cancer screening can decrease the threat of terminal lung cancer by 20%. SoonerCare offers a low-dose lung cancer screening benefit to members that can assist with early detection and a tobacco cessation benefit to aid with additional patient support.

Because we also know that patients who have diabetes and smoke are more likely to have complications that are worse than people who have diabetes but don’t smoke, SoonerCare also offers diabetes self-management education and support. This assists members with challenges and works to provide the necessary knowledge and tools to better manage their diabetes.


Chronic Hepatitis C Virus Treatment Coverage

Mavyret® (glecaprevir/pibrentasvir) is currently SoonerCare’s only preferred direct-acting antiviral medication for treatment of chronic hepatitis C virus. It is available without prior authorization through an ongoing initiative by OHCA.

Other direct-acting antivirals follow the PA criteria and require three forms to begin treatment.

  1. The medication-specific PA form (e.g., Harvoni®, Pharm-33)
  2. Hepatitis C Therapy Intent to Treat Contract form (Pharm-28)
  3. Hepatitis C Therapy Pharmacy Agreement form (Pharm-29)

Continuation requests for each refill must be submitted using the Hepatitis C Therapy Continuation form (Pharm-30).

Prescribers should follow current guidelines and Mavyret prescribing information when initiating testing for chronic hepatitis C, selecting an appropriate treatment regimen, and performing all necessary monitoring. Before treating, prescribers should verify the member's SoonerCare/SoonerSelect eligibility is in effect long enough to complete treatment. All patients receiving chronic hepatitis C treatment should be monitored for compliance and counseled regarding the importance of compliance and finishing treatment.

How to Help Your Patients Quit Tobacco

Do you talk with your patients about the risks of tobacco use? Nearly 50% of adults who smoke don't receive advice from their physician to quit. Here’s how to have that conversation, and how to recommend the Oklahoma Tobacco Helpline for FREE quit services.

The 5 “A” Tips to Help Your Patients Quit Tobacco:

  • Ask about tobacco use at every visit. This consistent reminder will help your patients realize the severity of their tobacco use.
  • Advise them to quit. Recommend the Oklahoma Tobacco Helpline as a free resource.
  • Assess their willingness to quit. Even if they’re just thinking about quitting, the Helpline’s free coach support can help them.
  • Assist with their quit journey. Everyone quits a little differently. The Helpline has several services — including free patches, gum or lozenges, encouraging texts and online services — that can help.
  • Arrange a follow-up. Schedule a time to check in on their progress and encourage them to use the Helpline’s dashboard to track progress between visits.

Your patients trust you. Your advice, combined with the free nonjudgmental support from the Helpline, can make a lifesaving difference. Encourage your patients to sign up to quit.

Free Services and Materials from the Helpline

One of the free services offered by the Oklahoma Tobacco Helpline is nicotine replacement therapy in the form of patches, gum or lozenges. If your patients are struggling with tobacco cravings, patches, gum or lozenges can help make quitting easier. No prescription needed.

You can also stock up on free Helpline materials for your office. Find products for your office like pens, brochures and prescription pages.

Is proof of an existing PA required for a transfer-back PA submission?

Transfer-back refers to a situation when a member has eligibility with a SoonerSelect contracted entity (CE) and then loses eligibility but remains eligible with SoonerCare. In such situations, if a PA was approved for services with the SoonerSelect CE, upon transfer-back to SoonerCare, this PA can be continued. The approved PA can be continued with SoonerCare for up to 90 days or the original end date of the CE approval letter, whichever period is shorter, when requirements for a transfer-back PA submission are met.

Documentation requirements for a transfer-back PA submission for speech therapy can be found at SLP, and for occupational and physical therapy at PT/OT. Coverage of services is limited to the specifics of the SoonerSelect CE PA approval letter. Due to OT and PT services utilizing the same CPT codes, the CE approval letter must state whether the service is approved for OT or PT services.

Transfer-back situations usually occur at the beginning of each month. As retroactive approval is tied to the specifics of the SoonerSelect CE PA approval letter, it is recommended to routinely check eligibility and timely submit a PA request.


Vaniecia: An ABLE Tech Success Story

Vaniecia sits in her power wheelchair in her home. Two Device Reutilization Program staff members stand behind her. They all grin at the camera.

Vaniecia, a resident of Oklahoma County, recently acquired housing with the assistance of the Oklahoma City Fire Department Community Advocacy Program (CAP), part of the OKCFD’s Mobile Integrated Healthcare initiative. CAP provides alternative assistance for individuals who frequently utilize 911 services. When fire crews identify such cases, referrals are made to CAP, where the team conducts a comprehensive assessment of the client's needs to help them find support and solutions.  

With a broken motorized wheelchair, Vaniecia couldn’t leave her bedroom. Thanks to the program, she was connected to Oklahoma ABLE Tech’s Device Reutilization program, where she applied for and received a power wheelchair. She can now go to the doctor and is very grateful. “I love the OKC Fire Department and ABLE Tech for helping me,” Vaniecia said. 

Oklahoma ABLE Tech staff retrieves donated equipment; sanitizes and refurbishes devices; works with vendors to repair the equipment, if needed; and reassigns durable medical equipment (DME) or devices to eligible Oklahomans, regardless of income.
