H.E.L.P Check 2013

Dear HealthChoice H.E.L.P CHECK Participant,

Earlier this year, you registered to participate in 2013 H.E.L.P CHECK . To ensure you receive your $200 incentive payment, you must complete your free preventive care visit by December 31, 2013.

Based on your age and the type of preventive care visit, the physician’s office must use one of the following codes when billing for these services:

• 99385 – Initial preventive medicine, new patient age 18-39 years
• 99386 – Initial preventive medicine, new patient age 40-64 years
• 99387 – Initial preventive medicine new patient age 65 years and over
• 99395 – Periodic preventive medicine established patient 18-39 years
• 99396 – Periodic preventive medicine established patient 40-64 years
• 99397 – Periodic preventive medicine established patient 65 years and older

Following the successful completion of a preventive care visit, you will receive your $200 incentive payment. Please allow 30 days for processing. If you recently completed the program requirements, please disregard this notice.

Don’t Overlook Your Check: Watch the $200 incentive check is mailed in the same type of envelope as your Explanation of Benefits (EOB) statements.

All program requirements must be met by December 31, 2013.
