Implied Consent: Impaired Driving Accountability Program (IDAP) Implementation
Implementation is Effective November 1, 2019!
Good Morning,
Senate Bill 712, the Impaired Driving Accountability Program (IDAP) becomes effective on November 1, 2019. The following are changes for all officers, deputies and troopers. In addition to the Officers Affidavit of Notice of Revocation/Disqualification, the program requires a new form, “IC/IDAP Notice 20191101” to be signed and issued to the arrestee. This form also has the new implied consent reading outlined in a “box”. The box is the only part of the form that must be read to the arrestee.
The following bullet statements are from the Department of Public Safety Legal Division.

Officers should read only what is in the box. This is the implied consent test request. Paragraph 7 is different from previous versions.

Officers should fill out the form and give a copy to the arrestee and submit a copy with the Officer’s Affidavit and Notice of Revocation.
The name of the Arrestee and Drivers License number or set-up number and state of issue will be printed on the top of the IC/IDAP Notice 20191101 form.
The Officer will hand serve this document to the arrestee, the date of service is printed onto the bottom of the page.
The Board of Tests has revised the handwritten “Officer’s Affidavit and Notice of Revocation/Disqualification” BOT Form AFF03 11/2019. The language has been modified and format cleaned up. A copy will be attached to this email. Please replace all previous handwritten refusal forms with the new form.
The new forms will be hosted on the Board of Tests Website beginning on November 1, 2019 at www.ok.gov/bot under the “Announcements” section.
