Home- and Community-Based Services for Older Adults and People with Disabilities
The Ohio Department of Medicaid, Bureau of Long-Term Services and Supports is pleased to provide new resources about Medicaid Home-and Community Based Services (HCBS) programs for older adults and people with disabilities.
Ohio offers four HCBS programs for older adults and people with disabilities who meet a nursing facility (NF) level of care. Each program offers varying services and supports for individuals to reside in their community rather than a nursing facility. These programs are called Assisted Living, MyCare Ohio, Ohio Home Care and PASSPORT.
Individuals and community stakeholders can compare eligibility, administration, and services for each program by referring to the Ohio Medicaid HCBS Nursing Facility Level of Care Programs resource. You can read this document here: HCBS Nursing Facility Level of Care Programs.
More information about all Ohio Medicaid HCBS programs can be found on the HCBS web page at this link: Home- and Community-Based Services | Medicaid (ohio.gov).
For individuals and community stakeholders seeking information on MyCare Ohio, along with guidance on moving through the MyCare Ohio process, a new resource has been developed called MyCare Ohio Process. The document outlines communication and information sharing expectations when members are moving between MyCare plans or the Ohio Home Care, PASSPORT, or Assisted Living waivers.
