 Deer Park to Host Optional Vaccine Clinic for Children Ages 5-11 on Tuesday, November 23
Deer Park Schools will be hosting an optional vaccine clinic for children ages 5-11 at Amity Elementary on Tuesday, November 23rd from 4:30-7:30 p.m. Any parent who would like to have their child vaccinated may attend this walk-in clinic. Children will be receiving the first of two Pfizer vaccine doses that will be given three weeks apart. Appointments are not necessary. All parents that choose to have their child vaccinated must fill out a minor consent form which is attached.

- A flyer with a list of pediatric vaccine providers, information about an ongoing town hall Q&A series for parents, and a QR code that scans to the webpage that has a full list of the pediatric vaccine providers in the region and their contact info for scheduling.
- A FAQ with information for parents about the side effects of the COVID-19 vaccine, safety and effectiveness, and dosage.
