Final Forms Notes
Parents,
We are still working to get all of our final forms signed off on for the 2019-2020 school year. It is important that you go into the system and at least sign off that nothing has changed. We use this information in the event of an emergency so that we know who to contact or preferences on local doctors. We also need this information in the system for students to continue to participate in fields trips and our after school enrichment series.
Please take time to get into the system and get this done. Here is the link if you need it: https://deerpark-oh.finalforms.com/
Thanks, Josh
