Vaccine Availability: K-12 Notice of Intent and Ohio Department of Health School Survey 


At a press conference yesterday, Ohio Governor Mike DeWine announced that schools wishing to receive COVID-19 vaccines for adult personnel must complete a K-12 Notice of Intent signifying that district’s (or school’s) commitment to returning to in-person or hybrid learning by March 1, 2021.

The K-12 Notice of Intent form is available online and must be signed by the superintendent or chief administrative officer of the district or school (please submit only one form per school district).

Completed K-12 Notice of Intent forms must be submitted by Monday, Jan. 18, 2021, to schools@odh.ohio.gov.

Additionally, earlier this week, schools should have received an email containing a link to a survey from the Ohio Department of Health. Data from this survey is necessary to assist health officials in planning Phase 1B vaccine distribution to school personnel.

If you have not yet completed this survey, please do so by close of business today, Friday, Jan. 8, 2021.
