For Your Awareness
Forms Transitioning to DCY
The Department of Children and Youth (DCY) was established on July 4, 2023. Since then, staff have been working to ensure all processes and programs remain fully operational during the transition process. DCY teams have been reviewing and updating all forms and documents.
The transition from the Ohio Department of Job & Family Services (JFS) forms to DCY forms is occurring in phases, with phase one focusing on those forms transitioning from JFS. Forms formally identified as JFS forms will become DCY forms on August 27, 2024. The form number of the form will remain the same. As a result, forms will change from a JFS XXXXX format to a DCY XXXXX format. For example, the KGAP Rack Card currently JFS 00152 will become DCY 00152.The form number referenced in Ohio Administrative Code rules will be updated as individual rules are updated.
Regardless of whether a form number begins with a JFS or DCY in a rule, the form can be located by searching by the form name or the form number on Forms Central. Any forms incorporated in an automated system will also start to transition from JFS to DCY as system updates occur.

Family Support Specialist Announcement
Ohio Family and Children First (OFCF) is happy to announce the four Family Support Specialist (FSS) program grantees for State Fiscal Year (SFY) 25. This program is funded via a long-standing agreement between OFCF, Ohio Department of Mental Health and Addiction Services, and Ohio Department of Children and Youth. Representatives from each of these state departments discuss and agree on programming deliverables, requirements, and funds allocated to this program each biennium.
The Family Support Specialist (FSS) program was developed to ensure families who have children with needs from multiple child-serving systems are afforded the opportunity to work directly with family mentors who have lived experience. The goal of the program is for families to feel better equipped to advocate for their child and family moving forward.
The SFY25 FSS regional awardees are:
- NAMI Lake County (Great Lakes Region)
- Stark County Educational Service Center (Northeast Region)
- Jefferson County Educational Service Center (Eastern Region)
- Butler County Educational Service Center (Southwest Region)
Each selected grantee will onboard at least one Family Support Specialist who will serve families within their region. The Family Support Specialist will guide and support families to obtain needed information while also encouraging the family to have a voice in their child/ren’s services and supports.
If you have any questions regarding the program, please send an email to OFCF@childrenandyouth.ohio.gov. In the subject line of the email, please refer to the Family Support Specialist program.
