State of Ohio to conduct a Dependent Eligibility Verification Audit
The State of Ohio continuously seeks to balance the demands of increasing health care costs while maintaining high quality benefits for you and your family. One way of protecting, strengthening and preserving these benefits is to ensure only eligible dependents are covered under our plans.
The Ohio Department of Administrative Services, in partnership with Secova, will verify dependent eligibility beginning September 11. You will be required to submit documentation for every dependent enrolled in the state's employee health benefit plans by November 10. Dependents who do not have the required eligibility documentation submitted will be removed from coverage upon completion of the eligibility verification process.
Enrollment or continuation of an ineligible dependent on an employee’s plan may result in loss of benefits, disciplinary action, and/or repayment of claims.
It is essential that employees receiving State of Ohio medical, dental, or vision benefits (including dental or vision benefits under Union Benefits Trust) submit all supporting documents to avoid losing benefits in 2026. Look for further communications in the near future with more details before the verification process begins in September.
How can you prepare for the Dependent Eligibility Verification? Gather valid copies of necessary documentation, such as your marriage certificate, dependent birth certificate, court documents related to adoption, guardianship or foster care, and/or other types of documentation that verify the eligibility of your dependent(s). To view the documents needed for each relationship type, go to DAS.Ohio.gov/Eligibility and select Change in Status/Qualifying Events Matrix.
- How to get copies of marriage certificates (or the document the county certifies, e.g., Abstract of Marriage) or birth certificates:
- How to get copies of court ordered custody agreements (adoption, legal guardianship, etc.)
- Court documents can be requested from the domestic relations division of the common pleas court, at the location the court order originated.
Documents may be obtained on the same day in some locations, where others can take two to five weeks to respond to mail-in or online requests. Please plan accordingly to have your documents ready and submitted before the completion of the eligibility verification process.
Sincerely, Ohio Department of Administrative Services
