Complete your biometric screening, earn rewards

A biometric screening is a quick and convenient way to check in on health measures such as cholesterol, glucose, and blood pressure. Knowing where you stand is an important first step toward improving your health and well-being. 

With dozens of screening events scheduled at state worksites, now is the time to make your appointment as spots are filling up quickly. For details about how to schedule an on-site screening, visit the Take Charge | Live Well Biometric Screenings page.

Get rewarded for taking healthy steps! Getting a biometric screening at a state worksite or having your doctor complete and submit the Physician Form (directions for submitting your form) are a part of three steps needed to get your incentive through the Take Charge | Live Well program.

To earn a reward:

• Take the Health Risk Check Survey on the Personify Health Hub.
• Get your biometric screening.
• Complete a Next Steps Consult call through Personify Health or get a preventive care screening.

All three steps must be completed by December 31, 2025. Then, you can earn $1,000 and your spouse can earn $350.
