Vaccination Reward for IAFF Local 67, FOP Lodge #9, AFSCME Local 1632, and CWA Local 4502

Attention members of the IAFF Local 67, FOP Lodge #9, AFSCME Local 1632, and CWA Local 4502

The City is offering a $500 Reward to full and part-time employees, hired on or before April 30, 2022, in the IAFF Local 67, FOP Lodge #9, AFSCME Local 1632, and CWA Local 4502 for being fully vaccinated against COVID-19 on or before December 31, 2021.

To be eligible for the $500 Vaccination Reward, an employee must have received one of the following:

one (1) dose of the Johnson & Johnson/Janssen vaccine, or
two (2) doses of the Pfizer-BioNTech vaccine, or
two (2) doses of the Moderna vaccine

Boosters are not required.

Employees must present a copy of their original CDC COVID-19 vaccination record card to their designated department or division human resources office on or before April 30, 2022. The CDC COVID-19 vaccination record card is a medical record and any retained copy shall be kept in the employee’s medical file (separate from the personnel file).

Payment for the Vaccination Reward will be made on May 19, 2022.
