 New Provider Enrollment Inquiry form (MEDCO-11)
BWC’s provider enrollment unit wants to assist you quickly! To help us standardize our intake format for your questions, we created a new form for our area, the Provider Enrollment Inquiry (PDF) (MEDCO-11).
This form highlights our three most received questions and has space for other questions you may have. Please read the directions on the form to learn how we respond.
The MEDCO-11 is available on the Forms and Publications section of our website, and you may email it once you save it to your computer. As of July 1, we are only accepting this form for all provider enrollment inquiries. Thank you for working with us.
 Help injured workers and grow your practice with HBAI services
BWC is recruiting providers to deliver Health and Behavior Assessment and Intervention (HBAI) services — an opportunity to support injured workers and boost revenue.
What are HBAI services?
HBAI addresses social and behavioral barriers that may delay recovery, such as:
- Inadequate coping skills
- Fear of movement or re-injury
- Perceptions of injustice
- Catastrophic thinking
These services focus on behavioral — not mental health — factors and may include coaching or counseling.
Who can apply?
Eligible providers include:
- Psychologists
- Licensed counselors, therapists, and social workers
- Physicians (M.D., DO), chiropractors, PAs, CNPs, CNSs
- Occupational and rehab therapists
No extra certification is required if it’s within your licensed scope of practice.

After enrollment, notify managed care organizations (MCOs) of your availability. MCO contact info is on our MCO directory page.

 Pharmacy outpatient medication formulary reminders
A prior authorization request (PDF) must be submitted for:
- All medications in medical-only claims beyond 60 days from the date of injury
- All medications exceeding $3,000
