Reminder: Updated Forms OC-RR and OC-110A

This is a reminder that the updated Case Copies Request Form (Form OC-RR) and Claimant's Authorization to Disclose Workers' Compensation Records (Form OC-110A) are available for use. The updated forms may be completed and submitted electronically (see How to submit, below).

The previous version of Form OC-110A will be accepted through September 9, 2026. Requests received after September 9, 2026, must be made using the updated form. Submissions received on the previous version of Form OC-110A after September 9, 2026, will not be accepted. 

Any valid submission of an original, verified wet signature Form OC-110A on file as of September 9, 2026, will continue to be valid. It is not necessary to submit a new form.  

How to submit 

A signed and witnessed Form OC-110A and completed Form OC-RR can be submitted using one of the following methods: 

  • Web upload: WCB Upload Service Sign In  
  • Email: wcbclaimsfiling@wcb.ny.gov 
    • Note that forms may be completed, signed, and submitted electronically OR printed, completed, signed, and then photographed or scanned for electronic submission. Note that photos can only be submitted as attached files, not embedded within an email.
  • Mail: NYS Workers' Compensation Board Centralized Mailing Address PO Box 5205 Binghamton, NY 13902-5205 

A few important notes:

  • Valid submission of Form OC-110A permits the individuals or entities listed to receive information and/or copies of case documents but does not automatically trigger the sending of such documents. 
  • Requests for copies of case documents may be submitted along with or after Form OC-110A on Form OC-RR.  
  • Unrelated party-of-interest requestors authorized under Form OC-110A must submit all records requests using Form OC-RR. Alternate formats will not be honored. 

Claimants may revoke authorization in writing at any time by any Board-accepted submission method, including email. There is no prescribed format for revocation. 

Additional updates to Form OC-110A

  • Two separate individuals or entities can now be listed as the authorized recipient of records to facilitate when a requester uses a records-retrieval agent. 
  • A witness signature and witness contact information is now required. Submission of an original wet-signature form is no longer required. 
  • Instructions for signing as a power of attorney or other authorized representative have been expanded for clarity.
