Reminder: New form version required for Section 32 waiver agreement

This is a reminder that the New York State Workers' Compensation Board (Board) has updated the “Resolution of Outstanding Medical Bills” section of the Waiver Agreement – Section 32 WCL (Form C-32) and that parties should only be using the new (8-25) version of this form.

What changed

For cases resolved by the agreement, the updated form now requires parties to agree to a date by which all medical treatment bills for established sites shall or shall not be deemed the liability of the carrier, and set forth any applicable exceptions. Additionally, the checkboxes on the prior form labeled “The date of the WCL Section 32 hearing” and “The date of the Proposed Notice of Approval-Section 32 Waiver Agreement (PD-32 desk) becomes final” have been replaced with a single checkbox, “The date of the WCL Section 32 hearing, or the date the Proposed Notice of Approval–Section 32 Waiver Agreement (PD-32 desk) becomes final.”

Please note: As of Monday, November 24, 2025, the Board no longer accepts Form C-32 with earlier version dates.

Make sure to update your forms accordingly to avoid any issues.
