Important updates to C-300.5 and C-312.5 forms 

On April 7, 2025, the New York State Workers' Compensation Board (Board) announced updates to two forms:  Stipulation (Form C-300.5) and Agreed Upon Findings and Awards for Proposed Conciliation Decision (Form C-312.5). The newly revised forms are now available for use. Both forms now have fields to provide information on: 

• Claimant representatives 

• Substitutions and related fee requests/agreements 

• Whether the case has a pending appeal 

As noted in the announcement on April 7, 2025, older versions of the C-300.5 and C-312.5 forms will not be accepted after Monday, July 7, 2025.  

Although no additional substantive changes have been made to the C-300.5 and C-312.5 forms, the Board has revised both forms to allow parties to type an unlimited amount of text in the “terms of the agreement” field of each form. This will eliminate the need to submit an attachment containing the terms of the agreement between parties.  

Translated versions of these forms will be posted on the Board’s website when they become available.
