IMPORTANT Reminders Before You Book An Appointment
You must show proof of your eligibility
Residents 18 years and older, please be prepared to show one of the following forms of ID at Dutchess County-operated sites:
- Driver’s license or non-driver ID
- Birth certificate issued by a state or local government
- Current U.S passport or valid foreign passport
- Permanent resident card
- Certificate of Naturalization or Citizenship
- Life insurance policy with birthdate
- Marriage certificate with birthdate
You must be available for your second dose appointment
Moderna is a two-dose vaccine series. Second dose appointments will be scheduled on-site immediately following your first dose. You must be available to return to the same location approximately 28 days (approximately May 13th or 15th) following your first dose.
If you will not be available to return for your second dose, please do not schedule an appointment.
Other Vaccine Allocations in Dutchess County
The following are other vaccine providers scheduled to receive allocations from New York State this week. Contact the individual locations for more information; many already have waiting lists for appointments.
Pharmacies
Appointments must be made directly with the pharmacy locations.
- Benzer Pharmacy, Amenia – 100 doses (Moderna)
- The Medicine Shoppe Pharmacy/Fishkill Pharmacy – 100 doses (Moderna)
Hospitals
- MidHudson Regional Hospital – 1,200 doses (Moderna)

PLEASE NOTE: The above list includes vaccine allocation sent to various locations by New York State. There are numerous other locations receiving vaccine doses from the Federal government (CVS, Stop & Shop, etc.) For a list of pharmacies receiving Federal vaccine allocation, visit the CDC's pharmacy program website.
New York State Vaccination Sites
Appointments are available at the various NYS mass vaccination centers, which include SUNY Orange in Middletown and the Ulster County Fairgrounds in New Paltz. To register for appointments at the State sites, visit the State's "Am I Eligible?" website.
PRO TIP: Even if it says "No Appointments Available," click on your preferred site. There may be recently cancelled appointments available!
