Dutchess County Weekly Vaccine Update

Dutchess County

This week, Dutchess County Department of Behavioral & Community Health (DBCH) is slated to receive 1,500 doses of Moderna vaccine from New York State. We will be vaccinating those over 60 years old, individuals with health conditions, and essential workers at our JCPenney and Dover Point of Dispensing (POD) sites by appointment only.

We are also planning another pop-up vaccination site in Beacon next week utilizing additional Johnson & Johnson (J&J) vaccine. Further details will be shared as they are finalized when the vaccine doses are received from New York State. Additional pop-up sites are planned for Pine Plains and Hyde Park in the coming weeks.

We also continue to partner with area primary care providers to vaccinate homebound and medically frail senior citizens.

Dutchess County representatives are contacting eligible individuals for all of the above event appointments.

Eligible individuals are randomly selected via a computer-generated process from the current list of nearly 60,000 people who have completed the Dutchess County Vaccination Information Request Form OR in coordination with essential worker employers.

by phone or email to schedule appointments.

Reminders

 

You must show proof of your eligibility

Please be prepared to show identification and/or proof of your eligibility as follows:

Phase 1a and 1b:

  • Driver’s license OR
  • Pay stub OR
  • Employer-issued ID

Qualifying underlying health conditions:

You can now self-certify that you qualify for the vaccine due to an underlying health condition by bringing a signed copy of the Dutchess County COVID-19 Vaccine Certification of Elgibility Form OR you can bring a note from your doctor or pharmacist attesting to your eligibility.

 

When Will I Get an Appointment?

You are receiving this email because you completed the Dutchess County Vaccination Information Request Form. You will be notified when a vaccination appointment at a Dutchess County-operated POD vaccination site in Poughkeepsie or Dover is available for you. Vaccine supply is still limited and it may be several weeks or months before you are notified for an appointment.

You only need to complete this form ONCE.  If you make a mistake or need to update your information, please resubmit the form; only the most recent submission will be used when selecting individuals.

Those who are 60 and older, or those without Internet access, can call the Dutchess County Coronavirus Information Line at 845-486-3555, Option 1, to hear weekly vaccine updates. Then choose Option 2 to be connected with an Office for the Aging representative who can help get you on the notification list.

 

Already got your shot?

If you submitted information through our Vaccine Information Request Form, but no longer need to be contacted about COVID-19 vaccination appointments, we're asking you to please complete our new Opt-Out Form. You will be removed from our notification list so we can more quickly notify others on the list about appointment opportunities. 

Please do not use this form to update your eligibility for a vaccine! To update information you originally submitted, please re-submit the Vaccine Information Request Form.
