Dear DMV Customer,
Your New York State license/driving privilege was suspended effective 12/01/2023.
When you renewed your New York State Driver License ending in –– on ––, you did not provide proof that you passed your vision test and instead, you self-certified your vision. You must submit a vision test result immediately to terminate the suspension. It may take up to five business days for the vision test result to be processed and suspension cleared. You have three options:
- Visit your local Vision Registry provider (such as a participating pharmacy or eye care provider) or use an online test provider. Vision Registry providers can be found at dmv.ny.gov/vision-registry-locator.
- Have your vision screened by your doctor and have them complete a Vision Test Report (MV-619) form which can be found at dmv.ny.gov/mv619.
- Visit a DMV office near you where a vision screening can be performed. Bring this letter to a local DMV office and the test will be administered. DMV office locations can be found at dmv.ny.gov/offices.

Please Note: If you are suspended or revoked for a reason other than failing to submit your vision test result those orders will remain in effect and must be resolved separately according to the notices provided to you. An indefinite suspension is not eligible for a conditional or restricted license or privilege. If you drive while your license or driving privilege is suspended, unless you receive a restricted license/privilege from DMV, you may be charged with and convicted of a crime and be subject to a fine and/or imprisonment.
