On October 21, 2020, Commissioner Beyer hosted a call announcing the decision of the Department of Children and Families (DCF) to relaunch the Service Report as a tool to gather invaluable input regarding program utilization for the Department's contracted services. Per this email, DCF is releasing the Service Report to gather monthly data for April 2023 and information on the impact of COVID 19 on the program’s operations.
The purpose of the short questionnaire (Service Report) is to help DCF capture data regarding the level of service delivered by providers operating direct service, cost reimbursement contracted services. The Service Report will provide DCF with crucial, real-time information as providers continue to adapt program service delivery in response to the COVID-19 pandemic. It will be shared with DCF Leadership to inform the Department's understanding of how the child and family service system is functioning.
You are receiving this email because you were identified by your agency leader (ED/CEO) as the contact person to complete a Service Report and/or you completed a prior Service Report for your program. If DCF did not have a point of contact for a program, you are receiving this because you are on file as the agency leader.
Each agency leader must ensure that only one (1) Service Report is completed for each program.
Please complete the Service Report by 5 p.m. on Monday, June 5, 2023.

** Please note: DCF has updated the Service Report Guide and Frequently Asked Questions to include specific guidance on completing the Service Report for Family Success Centers, School-Based Youth Services, Displaced Homemaker Services, Domestic Violence Services, Sexual Violence Direct Services, Sexual Violence Prevention Services, Batterers' Intervention Programs (BIP), Domestic Violence Liaison (DVL) Programs; Home Visiting Programs; Visitation Programs; and Peace: A Learned Solution (PALS) Programs.**

All questions regarding the Service Report should be submitted to the DCF Helpdesk at njspirithelpdesk@dcf.nj.gov. Please include "April 2023 Service Report" in the subject line of the email to ensure a prompt response.
Request to Change Service Report Contact Person:
Agency Leaders (CEO/ED) (or a designee as long as the agency leader is copied) can notify DCF to change the designated Program Service Report contact person. An email request must be submitted to DCF no later than the 2nd Friday of each month to Officeof.ContractAdministration@dcf.nj.gov.
Please include “Service Report Contact” in the email Subject Line; and include:
- First Name
- Last Name
- Email Address
- Contract Number & Program Name
These updates will be made for the report that goes out at the end of the month. Any updates that come in after the 2nd Friday of the month, will not be incorporated in the same month.
Please note, the Service Report for May 2023 will be sent to providers on Monday, June 26, 2023.
