As the spread of COVID-19 in New Jersey has increased and concerns regarding variants continue to evolve, the Department of Children and Families is monitoring the presence of COVID-19 in DCF-funded Out of Home (OOH) programs.

Please only click the link if you have a new COVID concern in one of your programs. Once you open the survey, you will be asked a very short series of questions regarding the presence of COVID-19 in your program. Following completion of the survey, a DCF representative will connect with you to discuss further needs. This follow-up will occur on regular business days. In the event of an emergency during non-business hours, please follow the emergency reporting protocols that your DCF program lead has identified. Additionally, it is required that your program staff complete an Unusual Incident Report for any COVID-19 issues that impact service.
The Department recognizes the impact that COVID-19 has had on children, families, our staff, and partner programs throughout the state, and that impact continues to present significant challenges to the residential provider community. As a Department, we want to ensure we are providing as much support to you as possible. Awareness and early intervention regarding COVID is the first step in that support.
