The Food and Nutrition Program Referral Tool
The tool was developed by the NH Intra-Inter Agency Food Security Work Group which consists of staff from DHHS (Division of Public Health, Division of Economic Stability, and Division of Long Term Supports and Services), Department of Administrative Services, and Department of Education.
The resource provides information on the federal nutrition programs available in New Hampshire. It is meant for anyone working with clients or patients who may need support accessing food
BAAS is Releasing the Home & Community-Based Resource and Referral Tool
The tool is the outcome of a 2-year pilot project focused on the reduction of food insecurity and malnutrition amongst New Hampshire’s adults, older adults and adults with disabilities. This resource was developed in response to the needs expressed by BAAS staff, statewide service providers, and community partners. And, is the result of the collaboration between BAAS’ Nutrition Services Providers, Adult Protective Services, and New Hampshire Hospital’s Department of Food & Nutrition Services; along with input from community partners.
The initial goal of creating a state-level, community-based nutrition services resource & referral tool was expanded to encompass socio-economic supports and resources that impact an individual’s overall health and wellbeing. The end result is a tool that supports improved awareness and communication of services for both providers and the residents of New Hampshire.

Go from Overwhelmed to Organized: NHPBS Guides You Through Planning for the Future
DURHAM, NH (February 19, 2026) —Planning for the future is one of the most important, yet often most avoided, tasks we face. On Thursday, March 26 at 6 PM, New Hampshire PBS will host an exclusive live event designed to help attendees move from feeling “overwhelmed” to confidently “organized” about their estate plans.
SENIORITY AUTHORITY: Planning Your Next Chapter will feature Cathleen Toomey, host of Seniority Authority, joined by Attorney Ben Siracusa Hillman, partner at Shaheen & Gordon Attorneys at Law. Together, they will lead an engaging, practical conversation about the essentials of estate planning in a relaxed, welcoming setting at the NHPBS studios in Durham.
This is an interactive evening created to help participants learn how to protect their assets, avoid the headaches of probate and ensure their families are taken care of. Whether just getting started with estate planning or looking to update a will or trust, attendees will gain clarity on the key steps needed to make confident decisions about their future.

Money Follows the Person (MFP): Updated MFP Grant Recipient Transitions
CMS released a brief report providing updated data through December 31, 2023, on transitions of Medicaid beneficiaries from institutions to community living under the Money Follows the Person (MFP) Demonstration. The report summarizes progress under the Demonstration in transitioning eligible people to the community and increasing Medicaid support for community-based LTSS.
The report shows that, from the time transitions began in calendar year (CY) 2008 to the end of CY 2023, MFP grant recipients transitioned over 127,000 people to community living through MFP. In CY 2022 and 2023, two target populations, older adults and people with physical disabilities, represented more than three-quarters of all transitions. Across MFP grant recipients, the annual number of MFP transitions increased by 77 percent between CY 2020 and 2023.
Medicare Enrollment Periods
The annual Medicare Advantage (MA) Open Enrollment Period and the General Enrollment Period both run from January 1st through March 31st each year. During the MA Open Enrollment Period, beneficiaries enrolled in a Medicare Advantage plan as of January 1st may switch to another MA plan with or without drug coverage. Alternatively, the beneficiary may drop their MA plan and return to Original Medicare by enrolling in a Part D prescription drug plan. Additionally, individuals who missed enrolling in Medicare Part A and/or Part B when they were first eligible and are not eligible to enroll during a special enrollment period may enroll in Medicare during the General Enrollment Period. West Health
National Aging Readiness Dashboard
West Health, in collaboration with the Center for Health Care Strategies, has launched the National Aging Readiness Dashboard, an interactive tool that brings together aging-related data from multiple national sources into one platform.
The Dashboard consolidates national- and state-level data to help policymakers, government staff, health plans, providers, advocates, and other stakeholders identify trends, uncover gaps, and inform strategies that better support older adults. National and state-level data domains featured in the Dashboard include: • Policy activity about states’ multisector plans for aging • Demographics • Economic security • Health care coverage, access, and costs • Health status • Mental health indicators • Dementia and cognitive decline • Caregiving, including both paid and unpaid caregiving • Housing and homelessness • Transportation and its impact on aging in place
