The Every Woman Matters (EWM) Program, offers blood work and education for cardiovascular disease awareness, along with the breast and cervical services.
Along with height, weight and blood pressure, EWM covers blood work, following the US Preventative Services Task Force guidelines, checking cholesterol and blood glucose levels. The EWM Screening Card that your patients bring to their office visit offers guidance on the blood work that is offered.
If values are elevated, providers may request a recheck of blood pressure (BP), total cholesterol (TC), or A1c or blood glucose (BG). EWM will cover an office visit with the provider and the blood work for the recheck.
EWM clients may receive a recheck of blood pressures or blood work of total cholesterol and/or A1c/blood glucose if:
- BP systolic >140 , diastolic >90, or if lower, on a case by case basis;
- TC of >240mg/dl; and/or
- A1c of >6.4% or BG of >139
Providers are required to treat “alert” values at the time of the screening visit. Providers may request a follow-up recheck from EWM, which would be covered. Alert values are as follows:
- BP systolic >160, or diastolic >100;
- A1c >10% or BG >250mg/dl; and/or
- Cholesterol >400mg/dl
When EWM clients enroll, they are asked to fill out the Healthy Lifestyle Questionnaire (HLQ), which contains questions regarding personal medical history, diet and physical activity, smoking, safety and wellness. When the Screening Card is mailed back to them, the HLQ is also sent back so that they can bring it with them to their office visit so that they can share it with their provider.
On the EWM Screening Card, providers may also participate in “Cardiovascular Risk Reduction Counseling” with their EWM patient. The first column must be filled in completely (two BP’s are required), and in the lower box, counseling or a support service must be marked. Participation in this service is not only good for your patient, but also for your clinic. If completed, it can garner $15 for your clinic.
If you have a patient who is a smoker, you can refer them to Tobacco Free Nebraska’s Tobacco Quitline. You can take an active role with their referral by completing the form with the patient and faxing a referral, or completing the referral online, or even a live phone referral. A Quitline coach will then be in contact with the patient. You can check out more information on the Tobacco Free Nebraska website.
