Each month, North Dakota Health and Human Services (HHS) will do a spotlight on a 1915(i) provider to celebrate their contributions and innovative approaches to person-centered care. By sharing their stories, we aim to inspire collaboration, foster continuous improvement and strengthen our community's commitment to delivering high-quality services.
This month, we highlight Jeff Jacobson from Aspire Services, Inc.
I am a lifelong resident of Leeds, ND, but have worked in Devils Lake for the past 15 years.
I enjoy traveling and doing some hiking with my family. I am a huge sports fan, so I enjoy attending games or watching on TV all levels of a variety of sports.
- How long have you been a care coordinator (or doing this kind of work)?
I started out as a program coordinator with a developmental disability agency in 2009 and have been involved with that type of work on some level since.
-
Could you share what inspired you to become a 1915(i) provider?
I started our private agency in May with a partner with our goal being to provide supports to everyone with the need. Becoming a 1915(i) provider helped fill that gap to find another service to help people.
- How do you tailor your services to meet the unique needs of each member?
We spend a lot of time getting to know each person in the environment in which they are most comfortable. That helps us build a level of trust with them and really be able to implement individualized services tailored to their needs. We don't force people to fit in one box of service delivery, we really work hard to provide specific services for each person.
