Please stay up to date with your SFN 750 or health care license renewals. If a renewal is not received by the due date, your qualified service provider (QSP) enrollment will close and you will need to reapply. Renewal reminders begin eight weeks in advance, so it is best to start the process right away. We kindly ask that you not contact the QSP Hub to request an extension, as SFN 750 extensions will not be granted. Your enrollment will close if the renewed document is not submitted timely.
Training videos
The following documentation for each new employee is required when adding new employees to the “Manage Employee” section of the QSP Portal:
- Fraud, Waste, and Abuse Training certificate
- Government-issued ID
- Documentation of Competency: SFN 749 or health care license
- Driver’s license if providing driver services
- Other documentation may be requested based on specific service selections
Reminder: conduct monthly screenings of your employees. This applies to any person with 5% or greater ownership or controlling interest, and any agents or managing employees using the following federal databases. Maintain documentation of each screening in your agency’s personnel files, as this information may be requested during an audit:
- Social Security Administration (SSA) Death Master File
- Office of Inspector General (OIG) List of Excluded Individuals and Entities (LEIE)
- System for Award Management (SAM)
Required under 42 CFR § 455.436 (Federal database checks and screening requirements for Medicaid providers).

Therap 101 Every Wednesday at 11 a.m. CT
The QSP Quality and Compliance Workshop was a free, one-day event designed to support QSPs in strengthening their knowledge of program integrity, audits, documentation and compliance tools. The event was hosted by North Dakota Department of Health and Human Services, Adult & Aging Services, with presentations and support from the Medicaid Fraud Control Unit, the Medical Services Division, the QSP Hub and team members from Adult and Aging Services.

What is a medical foster home (MFH)?
Medical foster homes are private residences where the caregiver lives and
provides continuous care and supervision to veterans, with the assistance of
relief caregivers.
What can the caregiver do?
This caregiver can help a veteran carry out daily living activities daily, such as bathing and getting dressed.
