North Dakota Health and Human Services (HHS) has partnered with the QSP Hub to enhance the profession's quality through an exit survey. We would like you to share your insights and experiences by completing a short survey.
Your input will help shape our future strategies and enhancements. The exit survey will be sent out monthly to those who are no longer QSPs.
If you have any questions, contact the QSP Hub at info@ndqsphub.org or (701) 777-3432, 711 (TTY).
Thank you for taking the time to participate.

Get in-person help claiming your QSP account. Visit the QSP Hub for a schedule of statewide trainings.
A home and community-based services (HCBS) provider navigator FAQ is now available on our QSP webpage. This FAQ provides information about referrals, QSP services, fees and more.
The QSP enrollment and revalidation process is now online for individual QSPs only, including family home care and family personal care QSPs. Agency QSPs will be sent notices soon on how to claim their accounts.
Why should I claim my account?
All notices about your QSP enrollment will come out of the portal. If you don’t claim your account, you won’t be able to revalidate your enrollment when it comes due. All future QSP notices will be sent through the portal and you will no longer receive revalidation notices in the mail.
How long will it take to claim my portal account?
Just a few minutes.
What happens once I claim my account?
Look at the information and make sure everything is correct. Use the tools available if any changes are needed.
Once you are done, you will get notices when a task is due. These tasks may include revalidating your competency, license or enrollment. Make sure to look for email notices and follow up with any assigned tasks. Your enrollment will close if you do not follow up on tasks.
Do I have to use the portal to bill?
No. The portal is only for staying up-to-date with your enrollment and contact information. You can update your address, phone number or email, counties you serve, add or remove services or change your direct deposit account.
How do I claim an account?
Follow the link sent to you in your email.
- Log in to the portal through the ND Login portal.
- If you already have a ND Login account used on another ND site, use that username and password to log in to the QSP Portal.
- If you do not remember your ND Login username and/or password, you can reset them following the instructions on the ND Login page.
- If you do not have a ND Login account, follow the steps on the ND Login page to create an account.
- Upon the successful completion of the ND Login step above, you will be asked a few identifying questions that will grant you access to your account.
What if I didn’t get the email to claim my account?
If you do not have access to your email account or have questions, call the QSP Hub at (701) 777-3432, 711 (TTY) between 8 a.m.-5 p.m. CT, Monday through Friday or email info@ndqsphub.org.
Did you know, if you bill through Therap, it tracks unit utilization and claim statuses?
If you are unable to see this information within Therap, you likely never completed a SFN 583 – Electronic Remittance Advice (835) Enrollment form upon enrolling as a QSP. This form must be completed to notify the department if and when your 835 claims remittances are to be sent to a clearinghouse such as Therap. This information is not relevant for family home care and family personal care QSPs.
If you use Therap to bill and this form was not completed, you need to send a copy of the completed form indicating Therap as your clearinghouse to ndmmisedi@nd.gov.
By doing so, you will be able to see unit utilization tracking and claim statuses within Therap. You will also get technical assistance on claims, claim resubmissions and more!
If you have any questions, contact the ND EDI Help Desk at (844) 848-0844 or ndmmisedi@nd.gov.
The individual service plan (ISP) data module in Therap allows individual and agency Qualified Service Providers (QSPs) to track and document on a mobile device or computer the tasks they complete during client visits.
The ISP is a safe and secure place for QSPs to store and retrieve their service documentation. QSPs can access, download and print all documented records on the ISP. The Therap ISP data module meets state and federal documentation requirements.
Using the Therap ISP program is not required but is strongly recommended!
Therap and the QSP Hub will be offering live training for QSPs on Zoom, Oct. 30 at 11 a.m. CT and on Nov. 6 at 1 p.m. CT .

A training video about using the ISP function in Therap is available on the HHS QSP and the QSP Hub websites. The Therap support page also provides instructions on how to use the ISP function.
If more assistance is needed using ISP, please set up a call with Therap Support by emailing ndsupport@services.com

Records must be maintained for a period of 42 months from the close of the federal fiscal year (Oct.1-Sept. 30) in which services were delivered.
- Records must be kept, even if your status closes or the client passes away.
- Documentation must be created at the time of the visit. Creating documentation after or before to the visit day is not acceptable and could be considered fraudulent.
- Records cannot be copied or cloned with times, dates or months changed.
All records must include:
- Name and ID # of the client
- Name and ID # of the provider
- Full date of the service MM/DD/YYYY
- Location of the service and the start time and end time that includes a.m. and p.m.
- Number of units of service using the task name as listed on the authorization

If using Therap, ISP data can be used to document your service records.
