Please join us for our quarterly QSP Advisory Group meeting!
The quarterly QSP Advisory Group meeting is an opportunity for individual and agency QSPs to voice their opinions and hear updates. This advisory group is a foundational step in creating more of a voice for QSPs across the state. The group's purpose is to work towards making improvements for the QSP profession. Advocating and providing your voice in the advisory group will not only promote movement forward in improving the profession but will hopefully also attract more individuals willing to do this valuable work. This group will meet quarterly to discuss relevant topics, brainstorm solutions and provide input to various stakeholders.

An agenda will be created and emailed out one week prior to the meeting. Please email Daniall Deis at daniall.deis@und.edu for any agenda requests. All requests are welcome.
You must keep records of the services you provide. Failure to keep records may subject you to legal and monetary penalties.
Records must be maintained for a period of 42 months from the close of the federal fiscal year (Oct. 1 – Sept. 30) in which services were delivered. Agency records must be maintained for seven years.
Records must be kept, even if your status closes or a client passes away.
Documentation must be created at the time of the visit. Creating documentation after or before the visit day is not acceptable and could be considered fraudulent.
Records cannot be copied or cloned with times, dates or months changed.
All records must include:
- Name and ID # of the client
- Name and ID # of the provider
- Full date of the service MM/DD/YYYY
- Location of the service and the start time and end time that includes a.m. and p.m.
- Number of units of service using the task name as listed on the authorization
- Tasks performed using task name as listed on the authorization
If using Therap, individual service plan (ISP) data can be used to document your service records.
The referrals that are sent from the QSP navigators are confidential and cannot be forwarded to other people once it has been sent to you. If you receive the referral, the information is for your use only and is not to be shared.

Did you hear? HHS launched a portal for both agency and individual QSPs. The portal and the HHS QSP website are the new sources for important QSP information.
You will see more improvements when managing QSP enrollment in the coming weeks. Notices to claim a portal account will be emailed to agency and individual QSPs in the coming weeks.
