Survey of Meal Distribution During School Closure as a Result of COVID-19 (Coronavirus) for Week of March 23-27, 2020
School Nutrition Administrators:
As discussed on the conference call earlier, the survey that is being used to capture data that the Office of the Governor needs to be updated on a weekly basis.

If you do not have any changes, please let me know so that I can “copy and paste” the responses from last week.
As a reminder, the questions included in the survey are:
- SFA Name/SFA #
- SFA Type
- County where SFA is located
- Is the SFA acting as a Nutrition Hub
- Total number of volunteers being utilized for meals service, delivery, etc.
- Provide the following information based on the total number of sites that have operated this week
- # Schools provided meals ONSITE
- # Schools provided meals OFFSITE
- # Sites provided meals ONSITE
- # Sites provided meals OFFSITE
- Of the total number of sites indicated in the previous question, how many sites offered meals to students via the following delivery methods:
