REMINDER NOTIFICATION

Survey of Meal Distribution During School Closure as a Result of COVID-19 (Coronavirus) for Week of March 23-27, 2020

School Nutrition Administrators:

As discussed on the conference call earlier, the survey that is being used to capture data that the Office of the Governor needs to be updated on a weekly basis.

If you do not have any changes, please let me know so that I can “copy and paste” the responses from last week.

As a reminder, the questions included in the survey are:

SFA Name/SFA #
SFA Type
County where SFA is located
Is the SFA acting as a Nutrition Hub
Total number of volunteers being utilized for meals service, delivery, etc.
Provide the following information based on the total number of sites that have operated this week
